Summary The Transfer Office RN provides direct staff support as well as oversight to the VA Community Care program in planning, designing, integrating, implementing, modifying, and evaluating the effectiveness of program components and services to improve the quality of patient care and the appropriateness and timeliness of services provided. The VA Transfer Office RN is responsible for the facilitation for intra-facility transfers and admissions at the Ann Arbor VA Medical Center. Responsibilities Duties of this position include but are not limited to: - Accurately manage the Bed Management System (BMS) and other electronic patient tracking systems, throughout the medical center to manage internal bed management and transfer requests. - Collaborate with medical center departments to manage and resolve bed utilization issues. - Educate internal and external customers on admission/transfer process. - Participate and lead as needed the daily bed meeting for our medical center. - Update the inpatient roster for daily morning meeting. - Participation in interdisciplinary committees/teams/task forces focused on the facilitation of patient flow and organizational performance. - Utilize effective communication with inpatient wards/units on bed availability and keep abreast of emergency room admissions along with daily scheduled admission from our surgical area. Reports directly to the VA Community Care Nurse Manager for day-to-day responsibilities. Performs his/her duties independently on a day-to-day basis. This position potentially requires flexibility in schedule and assignments, as this is a 24/7 hospital. The incumbent must be poised and articulate in communicating with people of varied educational levels and varied ethnic backgrounds. Willing to work cooperatively as a member of a team in all assignments. Makes caring for the veteran his/her priority while in the work setting and demonstrates customer service principles in all aspects of work. Self-motivated in the pursuit of meeting professional work standards. Basic Life Support (BLS) required for RNs in direct patient care positions. Advanced Cardiac Life Support (ACLS) required in specific specialty areas. Qualifications Basic Requirements: English Language Proficiency. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical patient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
