Description

Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician's plan of treatment, taking into account individual patients special or age related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential.

Required Qualifications:

Bachelor's Degree - Physical Therapy; from an accredited institution or,

Master's Degree - Physical Therapy; from an accredited institution.

Oregon Physical Therapist License upon hire.

National Provider BLS - American Heart Association within 30 days of hire.

1 year - Clinical experience in a healthcare setting, preferably home health, or active enrollment in the Providence Benedictine postgraduate preceptor program.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
