Summary The State Veterans Home (SVH) Coordinator is assigned to work at the Alvin C. York campus of VA Tennessee Valley Healthcare System (TVHS), located in Murfreesboro, Tennessee. The position may entail travel into the community up to 20% of the time, including visits to SVH providers if needed, and visits to other TVHS locations to provide staff education/outreach. May be required to work an irregular tour of duty and/or weekends, depending on the needs of the service. Performs combined responsibilities of SVH VA Medical Facility Representative and SVH Per Diem Contact Representative. Ensures that staff understand and adhere to program directives, policies, and standards. The SVH VA medical facility representative is designated by the Director, VA medical facility of jurisdiction, and is responsible for: Managing the SVH program at the VA medical facility level. Coordinating communication between SVH management, Director, VA medical facility of jurisdiction, SVH VISN liaison and Executive Director, GEC. Responding to inquiries from SVH management and referring SVH management to the appropriate VA office for assistance. Assisting the SVH, SVH VISN liaison and Executive Director, GEC, as requested. Immediately notifying the Director, VA medical facility of jurisdiction and SVH VISN Liaison that a sentinel event, negative publicity or any situation resulting in immediate jeopardy (IJ) to residents' health or safety has occurred. This includes preparing and submitting the 18 to the appropriate level, clarifying any questions and continuing to update the SVH VISN liaison until resolution. Serving as an educational resource for VA medical facility staff employees and SVH staff employees regarding the SVH program. The SVH Per Diem Contact Representative is designated by the VA medical facility of jurisdiction and is responsible for: Providing guidance to the VA medical facility of jurisdiction and SVH staff regarding SHPDP applications and claims. Communicating SHPDP responsibilities and outcomes to State officials, the VISN BIM, the VA medical facility of jurisdiction Director and GEC. Ensuring that the processing and reconciliation of all documentation and data (including SVH admission applications) with each SVH, to include daily admissions, discharges, bed hold days of payment, bills of collection and the accuracy of payment processing actions as detailed in Appendix A, occurs by the 15th business day each month. Submitting workload and expenditure reporting for the SVH under their jurisdiction to GEC. See Appendix A, paragraph 9 for details. Verifying that invoices for per diem payments are correct, complete and reconciled with the SVH using the approved VA tracking tool. See Appendix A for details. Processing per diem payments to the SVH on a monthly basis. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments.
