About the Position
This position is primarily responsible for providing a full range of clinical social work services to residents of The View Alexandria and their families to promote general social and emotional well-being and to identify their social and emotional needs which may be related to medical conditions. All employees are responsible for extending the mission and values of the organization by understanding the needs of those they serve. Each resident, staff member and community member will be treated in a dignified manner with respect, kindness, and understanding. Key Duties
Interviews potential residents to complete a social history and cognitive assessment in determining level of care.
Provides follow-up Social Work visits to newly admitted residents and documents such visits within ninety (90) days after arrival in the resident's medical record.
Offers direct social work interventions to residents and their families and provides appropriate measures for helping residents in their personal adjustment, emotional and social well-being and accessing of appropriate facility and community referrals and resources.
Documents in the clinical chart appropriate conversations, meetings, and interventions with residents, families/POA, and staff concerning residents as required by Federal and State regulations and TVA policy.
Administers the Mini-Mental Status Exam, Montreal Cognitive Assessment, Geriatric Depression Scale, Brief Cognitive Rating Scale, or other comparable assessments as needed.
Achieves certification by the Virginia Department of Social Services (VDSS) to participate in the completion of the Universal Assessment Instrument (UAI) and development of the Individualized Service Plan (ISP) within 90 days of hire. Will complete these documents within the appropriate timeframes as put forth by the VDSS, maintain all associated records, and organize and document service plan meetings with the resident, the interdisciplinary team and family/powers of attorney, on an annual basis and as required when there is a significant change.
Coordinates services to residents of on-site nursing home (if applicable) with medical and nursing staff to include care plans, MDS, and documentation as required by the Virginia Department of Health. Collaborates with the interdisciplinary team to complete the Minimum Data Set (MDS) within 14 days of resident admissions, quarterly, annually, and as required by change in resident status, and updates information upon any change in resident status.
Counsels residents and families to enable them to discuss and understand their feelings, attitudes, fears and anxieties related to problem solving and to explore alternative solutions.
Coordinates clinical social work services with the hospice social worker for residents who are hospice patients.
Develops and maintains contact with appropriate community agencies and services to promote cooperation and facilitate related referrals.
Participates in the development and implementation of resident care policies as related to psycho-social aspects of resident care issues.
Participates in the weekly interdisciplinary team meeting and collaborates with interdisciplinary team members to assist with admission, transfer and discharge planning. Provides appropriate social work follow-up and completes the VDSS Discharge Notification and Statement form, Assisted Living Transfer Form, social services notes in EHR, and all other necessary documentation as required by Federal and State regulation and TVA policy.
Provides quality resident care by coordinating and facilitating conferences with families/powers of attorney and other interested parties to address problems of adjustment, treatment, and caregiving concerns.
Advocates for resident rights in cooperation with other facility personnel, attends resident council meetings and coordinates a yearly resident rights update for all residents.
Educates on and promotes the completion of Advanced Directives, as required.
Provides assistance and information to residents and families/POAs related to special issues (i.e., US Census, annual TVA tax information letter, voting information, etc.), as necessary
Develops and presents in-service training as required.
Provides Social Work orientation to interns and new staff members within the department, as necessary.
Provides support groups for residents, family members, and the local community when appropriate.
Job Requirements
A BSW or MSW degree from an accredited university; prefer a licensed clinical social worker (LCSW).
Must possess computer skills; prefer experience with electronic medical records.
Prefer to have at least three (3) years experience in social work with at least one (1) year in work with the geriatric population.
Must possess ability to deal tactfully and effectively with residents, staff, family members and the public.
