Summary This Senior Social Worker (Urgent Care) position is located in Daytona Beach, one of the Orlando VA Healthcare System's multi-specialty satellite outpatient clinics. The Urgent Care Clinic, like the Emergency Department setting, is less structured and involves walk-in/triage patients, those who are "unassigned" to Primary Care Teams, and patients who are more complex in nature than those presenting for routine care. Responsibilities The Urgent Care Social Worker serves as a Senior Social Worker and provides complex medical Social Work services to the Veteran population seeking treatment at the Urgent Care Clinic. Given the unpredictable nature of the Urgent Care Clinic, the Social Worker provides advanced clinical services, which include provision of high-quality case management, clinical treatment, advocacy, and coordination of linkages to other appropriate VA or community service providers and/or agencies as needed by the Veteran and/or caregiver. Duties and functions of the Urgent Care Social Worker include but are not limited to the following: The incumbent will develop an assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members and significant others. The goal of the assessment is to highlight the Veteran's strengths, limitations, and internal/external supports and service needs to optimize the Veteran's functional status. The assessment process includes completing all applicable clinical reminders. Responsible for developing a treatment plan and setting achievable treatment goals with the Veteran/family. Independently provides advanced-level psychosocial support to a special Veteran population with highly complex health and/or mental health problems and the provision of short-term, solution focused counseling with Veterans and/or their families. The purpose of such counseling is to deal with the psychosocial impact of coping with chronic/disabling illness(es), onset of a catastrophic illness. Demonstrates an advanced understanding of the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs and age associated illnesses and conditions. Provides a full range of advanced practice Social Work services to include case work, Measurement Base Care, individual, family and group counseling, discharge planning, case management and other services as appropriate. Provides education related to VA and community resources, entitlements, Advance Directives/Living Will and will refer Veterans/families to the appropriate interdisciplinary team member for identified health education needs. Responsible for assisting the Veteran/family in establishing the appropriate referrals based on the Veteran's preference or that of his/her surrogate decision-maker. Makes rapid assessments and developing crisis management plans for Veterans within the VA and community. Will address Advance Directives Discussion, Advance Directives, Goals of Care planning or conversations in accordance with the standards of their work area. Participates in coverage for Social Work services on the assigned unit/service, and when needed for other programs or teams both inpatient and outpatient as needed. Coordinates treatment and communicate with other Social Workers and community program coordinators to include NIC, CNH, and TCM, Behavioral Health Programs, Homeless Program, Suicide Prevention etc., to facilitate continuity of care for Veterans, to include emergent placement if needed. Documents independently in the medical records utilizing paper and/or electronic formats. Documentation includes but is not limited to, initial evaluations, suicide risk assessments, group notes, advanced-level care coordination, case management, advanced directives, clinical reminders, or initial treatment plans. Utilizes Veteran Video Connect (VVC) technology, or other approved virtual care platforms, as one service delivery method available to provide Veteran care. In the event of a medical or psychiatric emergency, staff will follow the protocol of the VA Orlando Healthcare System including completion of suicide risk assessments and Suicide Behavior and Overdose Reports. Work Schedule: Monday - Friday; 8:00am - 4:30pm OR, 8:30am - 5:00pm. Pay: Competitive salary and regular salary increases. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. GRADE DETERMINATIONS Senior Social Worker, GS-12 Experience and Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification. Senior social workers must be licensed or certified by a State at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Note: This position requires a Florida advanced practice level licensure for involuntary hospitalization under Baker Act. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Preferred Experience Ability communicate effectively with people of various backgrounds and cultures. Ability to coordinate services and provide case management. Ability to work in highly stressful work environment and provide crisis intervention. Ability to document independently, including comprehensive psychosocial assessment, treatment plans, diagnosis, and complex interventions.
