Job Summary and Responsibilities
As an APP Spine Program Navigator, you will serve as the primary point of contact for patients being referred into the Back Pain/Spine Program. You will be responsible for coordinating the care and services of the identified patient population across the continuum of care.
Every day, you will promote effective resource utilization and assume a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers. You will employ self-direction and accountability in adhering to evidenced-based care protocols for the specified patient population. This involves meticulous patient management, empathetic communication, and proactive problem-solving to ensure seamless, high-quality care delivery and support for patients with spine-related conditions and their families.
To be successful in your role, you will strategically navigate and coordinate the care and services for patients within the Back Pain/Spine Program, acting as the primary point of contact across the entire continuum of care. You will demonstrate exceptional leadership within a multidisciplinary team, proactively identifying and eliminating barriers to care, and meticulously adhering to evidence-based protocols to optimize clinical outcomes for patients and their caregivers.
Scope of Role
The Spine Nurse Practitioner Navigator's responsibilities include:
-Develops and leads performance improvement initiatives related to quality and patient, physician, and
staff satisfaction
-Demonstrates leadership and commitment to excellence in patient care and service
-In collaboration with the interdisciplinary team, develops, maintains, and monitors spine practice
guidelines, order sets, and clinical pathway based on evidence based medicine
-Develops and nurtures collaborative working relationships that create a sense of ownership in the spine
service line with hospital department leaders, orthopedic spine surgeons, neurosurgeons, as well as other
staff and specialists involved in the care continuum for spine patients
-Monitors patient care outcomes for spine services
-Maintains expert knowledge of the spine specialty field
-Promotes and supports physicians and other staff participation in collaborative practice
-Represents the spine service line in the community
-Identifies and facilitates the provision of spine education for nursing, allied health personnel, and
patients/families
-Serves as a role model, teacher, coach, and mentor for the healthcare team and community
-Assists with patient and family education and patient satisfaction
-Assists with patient flow throughout the care continuum
Position Duties
Patient Service 60%
-Completes intake of patients upon referral into back pain program
-Conducts virtual visits upon intake and as needed
-Collaborates care with specialties such as surgery, pain management, therapy, and other treatment
services on behalf of patient through referrals and active communication
-Facilitates and participates in care conferences as needed
-Acts as resource and advocate to patient, following them through continuum of care
-Function as senior clinical support person providing input into program development, information about
clinical services
-Assist in development and distribution of educational materials for patients
Quality 20%
-Assess patient's physical, psychosocial, cultural, and spiritual needs through observation, interview, review
of records, and clinical data. Participates in quality assessment activities, track compliance with clinical
standards, patient's response, and intervenese in a timely manner to ensure best outcome
-Design and implement process improvement activities, analyzing results to improve both the
performance improvement and training initiatives
-Monitors ongoing documentation of patient care and data collection
-Monitors compliance with regulatory guidelines
-Efficiently monitor concurrent and retrospective data collection by working in conjunction with program
manager to ensure data entry into designated databases
People 10%
-Conduct formal and informal training to enhance team knowledge and operational efficiency
-Incorporate point of care training whenever possible and appropriate
-Research and present pertinent clinical literature to staff members
-Provide in service on high risk/low volume issues
-Asks as professional role model for staff, demonstrating ethical and legal professional nursing practices
-Promotes positive relationships with physicians, staff, and the community
-Demonstrates solid teamwork and team leadership
-Fosters open communication, shared leadership, and mutual respect to accomplish goals

-Collaborates with Director of Neurosciences and Back Pain Program Medical Director in growth and
development of spine service initiatives
-Participates in CommonSpirit's Spine Council meetings and supports system wide initiatives

-Able to prioritize and complete tasks independently in a time efficient manner
-Effectively use time, personnel, equ9ipment, and supplies to provide high quality cost effective delivery of
patient care
-Adapt to readily changing technologies, policies, regulations, and census through priority setting and
flexibility

This position is primarily remote/virtual but covers: CommonSpirit St. Anthony Hospital,St. Anthony North Hospital, Longmont United Hospital, St. Elizabeth Hospital - Fort Morgan Must have the ability to travel to any of these sites as needed
Preferred Experience
Experience with pain management including chronic pain is preferred. Knowledge of local resources in Denver metro regarding surgical spine, physical therapy, chiropractic services, and other providers related to back/neck pain. Possess strong technical skills, proficiency within Epic and related documentation, ability to manage high patient volume. Background in spine-related quality metrics and related process improvement projects.

-Proficient with database and spreadsheet management
-Ability to speak effectively before groups of customers or employees of the organization
Master's Degree in Nursing as NP from nationally accredited postgraduate program
Minimum of two (2) years caring for the surgical spine patient
Minimum of two (2) year in of clinic experience
Registered Nurse
Board Certified as APRN in applicable sub-specialty
DEA Registration Number

$49.00 - $83.27 /hour
