The Diabetes Planner provides statewide leadership to improve the wellbeing and reduce diabetes burden and health disparities in Minnesota in collaboration with key stakeholders. This position works with department staff, partners, communities, health systems and payers, diabetes experts, contractors, and grantees to provide expertise and technical support to achieve the goals of the Diabetes and Health Behavior Unit, CDC grant-related work, and departmental needs. This Planner role will also support clinical work within the Arthritis program, which a part of the Diabetes and Health Behavior Unit.
Key Responsibilities:
Provide vision, leadership, facilitation, coordination, and consultation to support theidentification, adaptation and/or developmentof evidence-basedandcommunity informed approaches to improve health and reduce diabetes-related health disparities. Approaches include strategies, programs, and services that are inclusive and are culturally and individually appropriate. Approaches aim to increase the identification, education, and referral of individuals with prediabetes, diabetes, chronic kidney disease (CKD), arthritis and other chronic conditions to evidence-based self -management and prevention education programs. Strategies also aim to increase screening for diabetes complications.
Providevision, leadership,facilitationand consultationontheimplementationandevaluation of identifiedstrategiestosupport the2035 StatePlan and CDCgrant objectivesin collaborationwith key stakeholders.
Promote diabetes prevention and managementbestpractices, innovations,and evidence-based prevention and self-management education statewide through and multiple communicationsvehicles and collaborations.
Maintain a high degree of scientific knowledge about clinicalhealth care systems, patient treatment and education standards,and professionaleducation/training for prediabetes, diabetes,CKD, and chronicdisease at the state and national level. Maintain a high degree of scientific knowledge about socialdeterminantsof health and primary prevention of diabetes and related diseases.
In accordance with HR/LR Policy #1422 Telework (https://mn.gov/mmb-stat/policies/1422-telework.pdf) , this position is eligible for up to 50% telework for applicants who live in Minnesota or in a bordering state within 50 miles of the assigned work location. Candidates residing outside of Minnesota or in a bordering state within 50 miles of the assigned work location at the time of application must be able to relocate to Minnesota within 30 days of their start date. The telework policy may exempt employees who live more than 50 miles from their primary work location from the 50% in-office requirement, depending on job requirements, helping the State of Minnesota access top talent across the state.
This position is anticipated to end on June 29, 2028. Due to its temporary nature, this position can be ended at any time.
Minimum Qualifications
Three (3) years of experience working in a public health program related to health promotion, chronic disease prevention and control, and/or engaging with health care systems. Experience must include the following:
Experience counseling and referring adults who are at risk for chronic disease.
Experience providing educational resources to adults at risk for chronic disease.
Experience facilitating meetings with various stakeholders.
Experience working within evidence based programming
Minimum Qualifications to be assessed at interview:
Knowledge of theories, principles, and practice of public health, community and population-based health promotion and education.
Knowledge of clinical health care systems and current and emerging clinical care, adult patient treatment models, systems and policies.
Knowledge of the Diabetes Self-Management Education and Support (DSMES) program as an evidence-based program for diabetes management.
Familiarity with Approved Arthritis Evidence-Based Interventions (AAEBIs) physical activity programming such as Walk With Ease or Tai Ji Quan: Moving for Better Balance.
Degree Equivalencies:
A related master's degree may substitute for eighteen (18) months of experience; a related bachelor's degree may substitute for twelve (12) months of experience; and a related associate's degree may substitute for six (6) months of experience.
Preferred Qualifications
Ability to understand the impact that policy and systems changes can have on quality, delivery, and accessibility of clinical preventive services and individual's self-management education and support.
Ability to manage complex projects and initiatives, design work plans, envision solutions, evaluate results and convey outcomes to stakeholders.
Ability to edit, organize, and synthesize relevant information from diverse sources to create public health communications, professional educational resources, programs, plans and reports.
Experience with grant management activities such as contract development, defining deliverables and invoicing.
