The assessment highlights the Veteran's strengths, limitations, high risk factors, acuity levels, internal and external supports, and service needs to optimize the Veteran's functional status. Ensure that appropriate family support, home screenings and assessment are completed to ensure the Veteran has the necessary and appropriate supports for post-operative discharge. This includes ensuring a responsible adult is identified pre-operatively, and the Veteran and caregiver are educated about the post-operative care requirements and limitations. Provides comprehensive psychosocial case management and care coordination for Veterans in pre-operative clinics, associated outpatient specialty clinics, and inpatient surgical care teams. The social worker serves as the subject matter expert on VA and community resources related to the needs of the Veteran, family, and caregiver. Collaborates with other providers in the on-going assessment of the Veteran's health care needs. The social worker is responsible for educating the Veteran and caregiver of available services and assisting them in making appropriate referrals based on the Veteran's preferences or that of the identified surrogate decision-maker. Provide consultation to Veteran, caregiver, and staff regarding travel and lodging options and eligibility. The incumbent will ensure care coordination for pre- and postoperative travel and lodging is completed. Evaluates progress towards treatment goals and proposes changes in the treatment plan as appropriate. The social worker monitors Veteran's progress, maintains comprehensive documentation, ensures diagnosis and treatment of clinical disorders, shows discretion in decisions to adjust treatment, and makes recommendations for care changes to interdisciplinary treatment plan. Ensures provision of clinical and psychosocial services and is accountable for the overall effectiveness of the services provided. The social worker modifies services as appropriate to meet the treatment needs of Veterans and to promote efficient practice. The Surgical Care Social Work Navigators coordinate services with other departments and programs to ensure such services are complementary and comprehensive. Ensure Veterans have a post-operative discharge plan identified with the Veteran, caregiver, and surgical team. Assists in identifying the health education needs of Veterans, their families, and caregivers and for working with the interdisciplinary team to develop a plan to address health education needs. Provides the Veteran, family members and caregivers with initial and ongoing supportive counseling in individual, family, or group settings as appropriate. Supportive counseling and caregiver support services will focus on preoperative, perioperative, and post-operative procedures and client psychosocial needs. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Full Performance Level Assignments. This is the full performance level. Social workers at this level are licensed or certified to independently practice social work. Incumbents are assigned to all program areas, including but not limited to: inpatient or outpatient medicine, surgery, mental health, neurology, rehabilitation medicine, and geriatrics. Employees provide professional, independent social work services in the assigned area. Incumbent independently assesses the psychosocial functioning and needs of Veterans and their family members, identifying the Veteran's strengths, weaknesses, coping skills, and psychosocial acuity. In collaboration with the Veteran, family/caregiver, and interdisciplinary treatment team, the social worker facilitates the delivery of health care services. The social worker identifies family/caregiver stressors, conducts assessment and provides specific interventions. The incumbent provides case management and care coordination to facilitate appropriate delivery of health care services, incorporates complex multiple causation in differential diagnosis and treatment of Veterans, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. The social worker links the Veteran with services, resources, and opportunities, in order to maximize the Veteran's independence, health, and well-being. The social worker conducts timely assessment of at-risk Veterans in crisis to identify immediate needs, evaluate risk, and initiate safety plan as appropriate. The social worker provides interventions independently with Veterans and their families/caregivers who are experiencing a wide range of complicated medical, behavioral health, financial, legal, and psychosocial problems. They provide a range of interventions and treatment modalities which may include individual, group, and/or family counseling or psychotherapy. They independently formulate and implement a treatment plan including measurable, achievable goals identifying the Veterans' needs, strengths, weaknesses, coping skills, and psychosocial acuity. Social workers serve on committees, work groups, and task forces at the facility and VISN level or in the community. They provide subject matter consultation to colleagues and students on the psychosocial treatment of Veterans offering professional opinions based on experience, expertise and role modeling effective social work practice skills. The social worker establishes and maintains ongoing education programs for Veterans, community agencies, students, and staff, to facilitate understanding of social work interventions specific to the Veteran/Military population.
