THE POSITION

In this vital role, you will serve as an advocate for older adults within our county who are seeking comprehensive care management services. DESCRIPTION OF WORK
The Aging Care Manager 2 receives formal and informal training as required to become knowledgeable in agency rules and regulations. In this vital role, you will be assigned intake and assessment duties, ongoing care management responsibilities and specialized duties to include investigation of Older Adult Protective Services cases. You can expect to explain availability, eligibility and restrictions of Agency programs to consumers, family members or professionals and work in close cooperation with community resources. This position prepares case histories, completes reports and meets all deadlines. It will be essential to become proficient with agency computer systems and procedures and to attend trainings and informational meetings to remain apprised of program changes.

Work Schedule and Additional Information:
Full-time employment
Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with 60-minute lunch.
On-Call hours when required.
Employee may have overnight travel or out of county travel for meetings or training.
Telework: You will not have the option to telework in this position.
Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONS
Minimum Experience and Training Requirements:
Six months as an Aging Care Manager 1 or a County Caseworker 1; or
Successful completion of the County Social Casework Intern program; or
Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
An equivalent combination of experience and training.

You must pass a background investigation.
A conditional offer of employment will require a drug screening.
This position falls under the provisions of the Child Protective Services Law.
Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
