Overview
The Registered Nurse (RN) will provide all necessary care management services to applicable pharmacy, medical, and home care patients. The RN will be tasked with working collaboratively with operations and pharmacy staff to ensure appropriate billing and reimbursement. The RN will assist the clinical team in performing care plan reviews, adherence calls, therapy goal assessments, patient management program assessments, and device training. In addition, will communicate with patients, providers, and members of health care team to ensure optimal patient care and improved outcomes through interventions where applicable. The RN will perform digital contact with patients through the Reliance Rx patient management program for clinical touchpoints to assess efficacy of prescribed medication, compliance, and other factors related to patient care. The RN will adhere to applicable regulations, patient care standards, and the NYS DOH approved policies and procedures.
Qualifications
Associate's degree required; Bachelor's degree preferred. Current, active, unrestricted and licensed registered nurse (RN) required.
Four (4) years of nursing experience to include two (2) years of direct patient care required, home care preferred.
Clinical knowledge of the health and social needs for the population served.
Proficient computer and Windows skills required, including MS Office.
Management experience of other nursing staff, preferred.
Experience in dosing and administration of intravenous and subcutaneous biologics, preferred.
Maintain current CPR/ACLS certification.
Maintain current Blood Borne Pathogens training certification.
Demonstrated transferable knowledge, skill and ability to complete job duties independently and proficiently.
Ability to assume responsibility and maintain confidentiality.
Flexibility in work schedules and assignments required.
Maintains appropriate means of transportation to administer services.

Essential Accountabilities
Assess the patient's broad spectrum of immediate and long-term needs through evaluation of the patient's social and medical history and vitals.
Develop a plan of care with providers of care and patients, to identified population along the continuum of care; staying within their scope of practice.
Provide clinical services within the scope of practices including but not limited to disease sate education, medication administration training, contacting patients in regard to medication compliance, and contacting prescribing offices with clinical interventions as needed.
Continual completion of training related to the skill of administering IV and SQ biologics.
Continual completion of required Continuing Education and clinical training per Reliance Rx Policy
Demonstrate proficiency in verbal and in-person training of medical devices.
Monitor proper billing and reimbursement for drugs, supplies, and time.
Safely and skillfully administer medications according to the prescribed dose, frequency, and intended use
Obtain/maintain necessary certifications
Provide ongoing assessment and documentation to monitor member's response to the plan of care; revises as needed based on changes in the member's condition, lack of response to the care plan, preference changes, transitions across settings, and barriers to care and services.
Perform and document initial and follow-up RELI-Rx assessments accurately and completely
Routine review and assessment of therapy goals for patients and other clinical touchpoints
Ensure compliance with regulatory standards as indicated; adhere to applicable local, state, and federal laws, as well as employer policies, governing all aspects of case management practice, including member privacy and confidentiality rights.
Maintain confidentiality and have proper training in dealing with protected health conditions and age groups.
Actively participates in project teams and medical management initiatives as needed.
Work collaboratively with Independent Health Case Management department on identifying needs and future opportunities.
Maintain a personal mode of transportation.
Maintain expenses related to travel.
Assist in the orientation of associates as needed.

Hiring Compensation Range: $80,000 - $90,000 annually
Compensation may vary based on factors including but not limited to skills, education, location and experience.
In addition to base compensation, associates may be eligible for a scorecard incentive, full range of benefits and generous paid time off. The base salary range is subject to change and may be modified in the future.
