Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. The Community Care Service (CCS) Nurse Manager (NM) demonstrates a high level of clinical expertise, leadership, and creative approaches to management of complex care and administrative duties. In addition, applies skills necessary to identify, coordinate, and implement discipline-specific education, leadership, professional staff development for all levels of staff. Actively develops and manages collaborative partnerships with physicians, other clinicians, and ancillary staff and across service lines. Provides oversight for the administration of leave, counseling, and disciplinary actions. Supervises and assumes accountability for the daily operations to include staff nurses (i.e., Care Coordinators (CC)) and administrative duties. Manages, supervises, and evaluates the clinical case management activities necessary to purchase patient care in the community in a manner that successfully integrates the community care with past and future VA care. Responsible and accountable for contributing to the establishment and implementation of evidence-based nursing standards of practice. Creates a positive work environment which enhances care delivery and professional development of clinically competent staff. Responsible and accountable for appropriate orientation, education, continuing professional development of nursing personnel, scheduling, staffing, evaluating, directing, and supervising nurses and ancillary staff. Ensures CCS CC competency with Office of Integrated Veteran Care (OIVC) Community Care programs to include, but are not limited to, Health Share Referral Manager (HSRM), Provider Profile Management System (PPMS), REFDOC, Electronic Medical Record (EMR), Enterprise Precision Scanning & Indexing (EPSI), Care Coordination, and Consult Tracking Manager (CTM+), etc. Provides vital input into decisions regarding VA Community Care. Complies and submits required reports in a timely manner to include: employee performance appraisals, proficiencies, performance improvement reports, certified time, meeting presentations, and/or committee reports. Develops and promotes consistent and high-quality care coordination for specialty care referrals utilizing both internal and community resources. Responsible for planning, developing, implementing, coordinating, and evaluating referral activities based on evidence-based models for the improvement of quality patient care and organizational processes. The CCS NM's primary concerns are with patient care, program development, consult management and staff training. Participates in the development of care delivery models and environmental design, ensuring evidence-based, accessible, affordable, and equitable patient care; Ensures compliance with accreditation and regulatory standards. Qualifications Basic Requirements: English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Education: Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)
