The ACT Team Registered Nurse functions as an integral part of the Assertive Community Treatment Team to provide intensive, recovery-based treatment for the highest risk consumers of mental health resources. The ACT Team Registered Nurse manages the medication system; documents medication treatment; communicates and coordinates with medical providers; engages in health promotion, education, and provider training to other team members; and assists consumers with understanding medications prescribed and the importance of taking medication. Essential Duties and Responsibilities Work collaboratively as part of the ACT Team in order to provide comprehensive care that meets the individual needs of each client enrolled in ACT; Assess client health daily and respond appropriately to any noted concerns (i.e. medication side effects, withdrawal symptoms, general health changes, etc.) Monitor client use of medications, changes in medications, and assess for efficacy, safety, and/or side effects of medications; Collaborate with all providers involved with clients health care and provide feedback to providers as needed; Link client to appropriate healthcare providers or specialists and follow-up with all referrals and recommendations; Provide health and wellness education to the client, the clients supports, and other ACT Team staff members; Participate in the formal training and development process with administrative staff and consultants as well as ongoing professional development and continuing education; Attend daily team meetings, individual supervision, and group supervision as scheduled; Initiate and maintain relationships with community resources, outside human service agencies, and other stakeholders as needed; Commit to ensuring fidelity to the ACT model and ongoing quality improvement efforts; Complete all required documentation accurately and on time; Other duties as assigned. Must be Registered Nurse in PA Subject to Reference and Background checks. SIGN-ON BONUS

$2,500 Sign on Bonus for qualifying positions. Must work full-time 40 hours per week. The agreement will be reviewed and discussed upon hire. Full Time Benefits:
