Summary The Social Work Case Management Program Coordinator oversees clinical case management programs within Social Work Service at Providence VA Medical Center including Transition and Care Management (TCM), Primary Care Social Work, and Inpatient Medical Social Work. The Supervisor oversees all transition and coordination of care activities for Veterans receiving case management in these program areas. Fulfills supervisory duties for staff assigned to TCM, PACT SW and Inpatient Social Work, including assignment of work, performance evaluations, selection of staff, training, recommendation of awards and/or advancements and where necessary, disciplinary action. Has the ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. Familiar with metrics within the Program areas assigned and is responsible for facilitating social work's support to meeting those goals including ACSC, Length of Stay and other metrics as they relate to TCM, Primary Care and Inpatient Care. Oversees the flow of care transitions between inpatient social workers and case managers back to outpatient care to ensure safe and successful transitions in care, and participates in Integrated Case management across service lines and disciplines as needed. Serves as the Point of Contact (POC) and Coordinator for the Transition and Care Management (TCM) Program and assumes all of the technical and administrative responsibility for management of this program. Provides administrative oversight of the TCM Program to include planning, establishing, and implementing policies and procedures; developing program goals and objectives; and monitoring, operating, and evaluating the functioning of the program. Follows Veterans Health Administration (VHA) policy requirements and develops local systems to identify and track post 9/11Servicemembers/Veterans (SM/Vs) served by the facility and community who belong in this special priority group. This may involve use of the OEF/OIF Department of Defense (DOD) Registry, Veterans Information System (VIS), Patient Treatment File searches, Veterans Health Information System and Technology Architecture (VistA), Care management Tracking and Reporting Application (CMTRA) and Computerized Patient Record System (CPRS) records, Northeast Program Evaluation Center (NEPEC) data reports and/or Classification reports, Patient Data Exchange, Network Health Exchange, and contacts within the community. Ensures that severely-injured and/or ill post 9/11 SM/Vs and other SM/Vs with identified case management needs are assigned to a nurse or social worker case manager and oversees or works closely with care management teams to include nurse and social worker case managers, the Visual Impairment Services Team (VIST) Coordinator, the Spinal Cord Injury (SCI) Coordinator, the Polytrauma Case Manager if applicable, and the Women Veterans Program Manager. Grade Determinations: Social Worker (Program Coordinator) GS-13: Experience/Education: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Assignment: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. At this grade level, social worker program coordinators oversee the administrative and clinical aspects of a major specialty treatment program, such as, but not limited to: Post Traumatic Stress Disorder, Substance Abuse Treatment Program, Home-Based Primary Care, Mental Health Intensive Case Management, Caregiver Support Program or Transition and Care Management Program, or a combination of programs located at one facility or multiple divisions of a facility. They are responsible for coordinating interdisciplinary staffing, work assignments, budget, treatment services provided, and admission criteria 19 for the program. The program coordinator may or may not have supervisory responsibilities. They have full responsibility for all phases of service delivery for a major specialty program. The program coordinator is responsible for the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. They are administratively responsible for the programming and operations of the program assigned, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. Program coordinators provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct program or service audits and complete designated clinical practice audits and reports, including productivity. Program coordinators oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement, and evaluating efficacy of improvement efforts.] Preferred Experience: Master's in Social Work, 5-7+ years in case management with proven leadership, strong knowledge of resources/regulations, data/software skills, crisis intervention, team training, and diverse population experience. Physical Requirements: Social Work Services are conducted at the facility, CBOC locations, Veteran Homes, community partners, and other locations. The work is generally sedentary. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations at VA facility or Veteran home. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders.
