Job Title: Health Information Management (HIM) Clerkand#10;Department: Health Information Management and#10;Reports To: HIM Managerand#10;Status: Non-Exemptand#10;and#10;Job Summaryand#10;The HIM Clerk is responsible for maintaining, organizing, and securing patient medical records in compliance with hospital policies, state and federal regulations (including HIPAA). This role supports clinical staff by ensuring timely access to accurate health information and assists with release of information requests.and#10;and#10;Essential Duties and Responsibilitiesand#10;Retrieve, file, and maintain patient medical records (electronic and paper) accurately and promptly.and#10;Process requests for medical records for patient care, audits, and quality review.and#10;Coordinate release of information to authorized entities (insurance companies, attorneys, government agencies) following HIPAA and hospital policy.and#10;Verify and log authorizations for record release; process subpoenas and court orders as directed.and#10;Answer department phone lines and respond to inquiries regarding medical records.and#10;Assist physicians with record completion and deficiency resolution.and#10;Scan and index documents into the Electronic Health Record (EHR) system.and#10;Maintain confidentiality of all patient information at all times.and#10;Support compliance with regulatory requirements and participate in quality improvement activities.and#10;and#10;Qualificationsand#10;Education: High school diploma or equivalent required; coursework in medical terminology preferred.and#10;Experience: Previous hospital or healthcare experience preferred; familiarity with EHR systems is a plus.and#10;Skills:and#10;Knowledge of health information management practices.and#10;Strong organizational and interpersonal skills.and#10;Ability to prioritize tasks and work independently.and#10;Proficiency in basic computer applications.and#10;Excellent attention to detail and confidentiality.and#10;and#10;Physical and Work Requirementsand#10;Ability to lift up to 25 lbs.
