Job Summary and Qualifications

The Case Management Assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions.

You will coordinate with RN case managers and social workers for needed support and will communicate updates on progress throughout the day.

You will provide clerical support in multidisciplinary rounds and will make referrals through the web-based referral system to the specific post-acute care (PAC) providers requested.

You will monitor patients with PAC referrals and intervene to expedite discharge, as well as prioritize workload to ensure a timely contact and intervention with PAC providers.

You will communicate with RN case managers/social workers regarding the status of PAC referrals, bed availability, and other barriers.

You will utilize Case Management systems, standards, and processes and will maintain knowledge of and actively develop relationships with PAC providers.

What qualifications you will need:

High School Graduate. Associate Degree preferred

Minimum 1 year of healthcare experience required

Medical knowledge and terminology required

Inpatient acute care case management experience required

Discharge planning background and experience required

Riverside Community Hospital is a large acute care facility with 517 beds, established in 1901.
