Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Responsibilities The Licensed Practical Nurse serves as a member of the multidisciplinary health care team with selected patients and provides care to them on a regular and recurring basis. The LPN completes assigned duties and responsibilities, which involve performing standard and specialized practical nursing activities and which require broad work experience. Recurring duties are carried out independently to include: Documenting care given, observations, and responses to care on the appropriate tools or forms. Assuring receipt of adequate nutritional intake either through observation of patient/resident, feeding the patient/resident, or properly positioning and checking placement of feeding tube. Preparing and administering prescribed medications (oral, topical, subcutaneous, within, intramuscular, and or intravenous) and performing treatments according to established policies/procedures. Observes physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviations from normal to RN or appropriate team member. Monitors intake and output, according to established policy and procedures. Integrate and apply aspects of the Joint Commission s National Patient Safety Goals as applicable to their area of practice. Integrates and applies pertinent aspects of the performance improvement program into his/her professional practice.
