Department: Outpatient Rehabilitation

Schedule: Full-time, some mornings and evenings required (see below)

Facility: Ascension Alexian Outpatient Center

Location: Schaumburg, IL

Sign-On Bonus: Up to $10,000.00

Salary: $79,939.65 - $116,912.25 or per year

Responsibilities

Evaluate and conduct medically prescribed physical therapy treatment programs.

Perform initial and on-going assessments of patient's condition.

Establish, revise and evaluate a plan of care which is appropriate to problems identified and involves the patient/family.

Perform therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.

Oversee activities of physical therapy assistants, students and other support personnel.

Requirements

Licensure / Certification / Registration:

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.

Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required. A valid license pending letter will be accepted until license is issued.

Education:

Bachelor's degree required.

Master's degree preferred.

Additional Preferences

Lymphedema, Pelvic Floor, Vestibular, LSVT Specialties Preferred.

This role a wide range of patients including Orth, Neuro, Pelvic Health, Cancer services, Vestibular, Occupational and Speech Therapy.

Availability to work the following business hrs.:

M-TH 7-8pm (2 evenings a week required)

F 7-545pm

Sat 730-11:30 (1 Saturday a month required)
