Summary This position serves as a Program Coordinator for the Homeless Engagement and Recovery Opportunity (HERO) program within Community & Residential Care Services (CRCS) for the Mental Health Service Line. The HERO Program Coordinator has broad program management responsibilities which include the operation and management of the 16 bed, 24x7 residential treatment program for Veterans who are homeless and trying to get back to work. Responsibilities Total Rewards of a Allied Health Professional Clinical Duties: Provides full range of advanced level clinical care to a complex population of patients with a wide range of clinical needs. Is a knowledgeable and experienced clinician, proficient in psychiatric diagnostic assessment, suicide risk assessment, crisis intervention, and crisis management. Provides comprehensive mental health and psychosocial assessment and diagnostic services for Veterans with a broad range of mental health issues. Provides education to patients and family members. Provides evidence-based psychotherapeutic interventions. Provides consultation to health care providers about assessment findings and treatment plan for patients. Develops, implements, and documents the mental health treatment plan for an assigned caseload of patients. Coordinates community-based services. Possesses general knowledge of Veterans' benefits and services relating to special programs, service-connected compensation, and non-service-connected pension. Facilitates referrals based upon Veterans' needs and eligibility. Independently collaborates in identifying and addressing ethical issues that impact patients and staff. Analyzes ethical/care issues. Develop working relationships with the community and VA providers. Refers Veterans to a wide variety of community and VA housing resources, maintains the relationships with those respective facilities/agencies, facilitates inter agency communication and care conferences as needed and provides discharge planning for Veterans that maintains a continuity of care. Complies with all Service Line documentation requirements. Adheres to all Facility and Service Line policies and practices for delivering clinical care. Achieves minimal clinical workload expectations as established by the Service Line. At the GS-13 level, social workers are licensed and function independently. Social workers at this grade additionally assume independent professional responsibility for their clinical assessment findings, patient care decisions, and documentation. Supervisory Duties: Performs the administrative and personnel management function relative to staff supervised. Will have established guidelines and performance expectations for staff, clearly communicated through the formal employee performance management system. Evaluates staff performance. Develop performance improvement plans, recommending personnel actions as necessary. Work to resolve staff complaints and grievances. Provides advice and counsel to workers related to work and administrative matters. Invokes disciplinary measures as appropriate. Reviews and approves or disapproves leave requests. Assures that subordinates are trained and fully comply with the provisions of safety regulations. Provides staff training and development at multiple grade levels. Conducts ongoing reviews and performance appraisals. Coordinates/tracks data related to productivity, timely provision of services, and appropriate documentation of workload. Recognizes gaps in workforce competencies and ensures supervised staff who require knowledge and skill development receive the training necessary. Certifies staff completion of all required annual and ad hoc training. Provides consultation/training to all stakeholders on the complex needs of Veterans with PTSD and SUD. Ensures staff supervised are aware of and adhere to all current VHA directives and meet relevant accreditation standards. Provides orientation and training for temporary staff working on the unit. Provides clinical supervision to unlicensed social workers working towards licensure. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. In addition to the experience above, the candidate must demonstrate all of the following KSAs. Skill in coordinating and implementing one or more specialty program(s), across the facility. Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. Assignment: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. At this grade level, social worker program coordinators oversee the administrative and clinical aspects of a major specialty treatment program, such as, but not limited to: Post Traumatic Stress Disorder, Substance Abuse Treatment Program, Home-Based Primary Care, Mental Health Intensive Case Management, Caregiver Support Program or Transition and Care Management Program, or a combination of programs located at one facility or multiple divisions of a facility. They are responsible for coordinating interdisciplinary staffing, work assignments, budget, treatment services provided, and admission criteria for the program. The program coordinator may or may not have supervisory responsibilities. They have full responsibility for all phases of service delivery for a major specialty program. The program coordinator is responsible for the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. They are administratively responsible for the programming and operations of the program assigned, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. Program coordinators provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct program or service audits and complete designated clinical practice audits and reports, including productivity. Program coordinators oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement, and evaluating efficacy of improvement efforts.
