Summary The VA Eastern Colorado Healthcare System,, Social Work Service is currently recruiting for one (1) Social Worker Program Coordinator, GS-0185-12, for Marcus institute for rain Health (MIBH)/Avalon Network at the Department of Veterans Affairs Clinic, in Aurora, Colorado. Responsibilities Total Rewards of a Allied Health Professional Major duties and responsibilities of the position include but are not limited to: Oversees the daily operation for the MIBH/Avalon Network/VHA partnership and programs, develops policies and procedures for program operation. Responsible for program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. Meets with Veteran/Service Member and family/caregiver providing direct patient care, education on VA health benefits and resources, and case management and care coordination. Utilizes and/or refers to both VA and non-VA resources across the continuum to address current medical, mental health and psychosocial needs. Collaborates with interdisciplinary teams across VHA health care systems to facilitate care coordination. Collaborates and communicates with various agencies and departments at the national, state and local level in ensuring the seamless transition of health care. Prepares reports, briefs, and presentations for VA staff at all levels, MIBH/Avalon staff, Congressional Staff, Community Organizations, etc., regarding the MIBH/Avalon VA process and specific mechanics of the program Independently manages the day-to-day operations of the MIBH/Avalon Liaison initiative and provides accountability to program effectiveness and modifications of service patterns to enhance customer service. Identifies gaps in processes and collaborates with MIBH/Avalon and VA to enhance these processes and procedures. Provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. Initiate and conduct service audits and complete designated clinical practice audits and reports, including productivity assessments. Develops, gathers, and applies new data as needed to successfully plan for and implement program goals and projections. Develops and implements transition policies and programs that are of vital significance and interest to the top management of VHA, VA, RMR/MIBH, Veterans Service Organizations, and Congress. Provides leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. Qualifications Qualifications To qualify for this position, applicants must meet all requirements by the closing date of this announcement. Basic Requirements: United States Citizenship. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Individual may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social work program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Knowledge of VA programs, services, resources.
