Summary This Licensed Practical Nurse (LPN) position is located at the Grafton VA Clinic in Grafton, ND. As a vital member of a Primary Care Patient Aligned Care Team, the LPN engages in a collaborative, patient-focused model that prioritizes wellness and disease prevention. Conduct quality controls on point of care lab testing machines and perform routine College of American Pathologists (CAP) proficiency testing for all test platforms. Perform a variety of critical care tasks, which include monitoring patients' vital signs, assessing pain levels, executing clinical reminders, performing blood draws when required, applying braces or splints, irrigating ears, collecting culture specimens, changing dressings, and removing sutures or staples, as well as conducting EKGs. Monitor and document the patient's physical state, emotional reactions, skin condition, fluctuations in vital signs, and personal habits, paying close attention to any indications of the patient's response to their illness and treatment, and ensuring all observations are recorded in the designated format. Respond to concerns of patients and visitors in a manner that is effective and improves customer satisfaction. Regularly inspect equipment, supplies, and containers to confirm that products are current and adequately stocked in accordance with unit procedures. Collaborate with PACT staff to create detailed health care and management plans for assigned patients. Completed work should need only a general review by a registered nurse (RN) or physician (MD/DO) for appropriateness and conformity with established policies/procedures. Ability to observe, identify, and respond to a patient's needs for care including medication, equipment-assisted care, and patient/family education. In organizing and delivering care, the LPN/LVN recognizes and considers emotional, socio-economic, and age-related factors. Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures.
