About the Position
Position is primarily responsible for seeking and coordinating admissions to the Health Care Center, maintaining census targets, developing relationships with Hospital Discharge Planners and verifying insurance as required. Develops and maintains other referral sources including area Nursing Facilities without a SNF, local ALFs, Physician groups, Insurance networks, GL at Home, and Goodwin House Hospice.
Position also provides support services to the Director of Nursing, Nursing staff, and residents of the Health Care Center. Is responsible for coordinating and completing all required documents for admissions to the Health Care Center; processing Nursing staff payroll biweekly and completing data input for resident admissions.
Job Duties:
Admissions
Coordinates admissions to the Health Care Center and obtains the following information: referrals for potential admissions; complete admission packet to include VIS sheet, medical records from hospital; information not available in the medical record; verifies insurance information; reviews information for required documentation, identifies attending physician for resident; and obtains various departmental approvals as necessary for admission.
Serves in public relations role with persons seeking short-term rehabilitation placement or long-term private pay in the Health Care Center; ensures that appropriate information is provided to inquiring persons; explains requirements and benefits to applicants
Develops relationships with hospital discharge planners. Performs insurance verification and pharmacy screens for potential applicants as needed for financial approval within an appropriate time frame
Responds to admission inquiries within 1 hour of receipt; includes but not limited to working with hospital social workers, families, and prospective residents.
Verifies Medicare eligibility of identified potential admission. .
Monitors all Goodwin House residents' status while they are hospitalized. Maintains contact with hospital case managers to coordinate the planned discharge.
Ensures rooms are prepared on the Health Care Center to meet the standards of Goodwin House and is ready to receive an admission. Coordinates with Facilities Management and Environmental Services and submits work orders as appropriate.
GHBC Liaison with Medicare Advantage and all HMO/PPO plans including pre-authorization, benefits verification per episode of care, regular insurance case manager updates (verbal and in writing), resident & family of benefits, and contract management.
Administrative
Serves as Payroll Coordinator for H&W including approval of Clinic and HCC payroll.
Serves as backup for submission of Payroll Based Journaling quarterly
Maintains Plan-of-Care schedule. Updates monthly Plan of Care list with admissions added to Plan of Care.
Orders office supplies monthly for HCC and Nursing Administration teams.
Disseminates mail to designated team members and residents.
Inputs census into resident register weekly and updates in real time. Disseminates the resident register weekly.
Job Qualifications
Bachelor's Degree preferred in Health Care field, Social Work, or other related field.
Must have knowledge of Medicare and commercial insurances, able to explain to resident/families what is covered by insurance and what costs will be private pay.
Must have general knowledge of medical field, diagnoses and what scope of treatment is provided at Goodwin House.
Must possess the ability to review the contract for admission with residents and families and answer appropriate questions.
