Summary The position is eligible for Education Debt Reduction Program (EDRP), student loan payment reimbursement program. Responsibilities The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) in Home Based Primary Care (HBPC) practices in Extended Care and Palliative Medicine Service to provide a full range of health care services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management. The major duties and responsibilities include, but are not limited to: Independently manages a panel of patients with acute and chronic medical, social, and behavioral conditions, with an emphasis on caring for older, frail, homebound patients. Visits patients in their homes in the community. Assesses, identifies, and diagnoses patients' general medical problems and defines the medical management. Qualifications Advanced Practice Nurse (Nurse Practitioner) Basic Requirements Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity.
