Job Overview

We are seeking a dynamic physical therapist to join our Flexible Workforce (FWF) team and provide orthopedic PT services to adolescent through adult clientele.

The FWF team provides staffing coverage across multiple outpatient orthopedic therapy clinics in the greater Twin Cities metro area. Preferred candidate will have mechanical diagnosis and therapy (MDT) training. Where MDT training and experience is limited, Fairview offers mentoring and development opportunities for professional growth.

In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.

FTE: 1.0, authorized for 80 hours per pay period.

Schedule: Monday - Friday 8:00am - 4:30pm variable start time +/- 30 minutes.

Position provides coverage at various locations across the greater Twin Cities metro including, Blaine, Bloomington, Burnsville, Eagan, Edina, Eden Prairie, Fridley, Golden Valley, Hugo, Maple Grove, Maplewood, Minneapolis, New Hope, St. Anthony, St. Paul, and Woodbury.

Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

Rehab's continuing education program offers 40+ continuing education courses per year at no cost to employees and further supports numerous external CE courses each year.

Responsibilities

The physical therapist partners with the patient and family while performing the Evaluation/Re-evaluation

Gathers pertinent data

Performs examination

Determines treatment diagnosis

Develops an individualized plan of care through collaboration with the patient and family

Analyzes data to identify needs and potential risks

Determines prognosis for recovery

Establishes individualized goals with patient and/or caregiver

Individualizes plan of care considering developmental level and cultural differences

Implements and modifies plan of care based on reassessment and patient response

Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care

Establishes appropriate discharge plan

Partners with patients and families to provide skilled intervention

Implements plan of care

Alters treatment/plan of care to reflect change in patient status and response to treatment

Assesses patient and/or caregiver learning needs and provides appropriate education

Required Qualifications

Physical Therapy License in the state of Minnesota or temporary license in the state of Minnesota

Basic Life Support (American Heart Assoc or Red Cross)

Preferred Qualifications

Doctorate of Physical Therapy

1 year experience

MDT Training

Compensation Disclaimer

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Hiring at the maximum of the range is not typical.
