What You'll Do

As a key member of our care team, you'll be the vital link between patients, families, and healthcare providers. Your day-to-day will include:

Acting as a trusted liaison between patients, families, and clinical teams

Maintaining accurate and timely medical records

Managing and restocking essential medical supplies

Supporting quality assurance initiatives

Assisting with onboarding and mentoring new team members

Preparing patient histories and organizing lab data

Providing clear follow-up instructions and advanced telephone triage

Participating in exciting research projects that shape the future of care

What You Bring

A current NYS LPN license in good standing

Strong knowledge of medical terminology and EMR systems (NextGen and Epic a plus!)

Experience in case management and patient coordination

A graduate of an accredited nursing program

A track record of reliability and professionalism

Schedule & Perks

Monday-Friday, 8 AM-5 PM(some flexibility may apply)

No holiday shifts

Pay and Benefits

The pay range for this position is $28.00 - $35.00/hr.
