Position OverviewUnited Health Services (UHS) is seeking a dedicated and patient-focused Physical Therapist Assistant (PTA) to join our Home Care team. In this rewarding role, you'll help patients regain strength, mobility, and independence by delivering therapy services in the comfort and familiarity of their own homes. Working closely with Physical Therapists, you'll carry out individualized treatment plans and support patients through every step of their recovery journey.

$22.89 - $32.76 per hour, depending on experience
Job Responsibilities
Delivers therapy interventions as directed by the supervising Physical Therapist, including patient preparation, instruction, and implementation of individualized treatment plans
Educates patients and caregivers on treatment goals, precautions, and home exercise programs to support independence and safety
Demonstrates proper use of rehabilitation equipment in the home and ensures it is clean, functional, and safely maintained during transport and use
Observes patient responses to therapy and reports changes or concerns to the Physical Therapist to support timely reassessment or plan adjustments
Accurately documents treatment sessions, observations, and progress in both paper and electronic systems, meeting all regulatory and agency standards
Submits billing and visit documentation in accordance with UHS policy, ensuring timely and accurate records for reimbursement
Participates in discharge planning by supporting caregiver education and offering insight into patient needs and readiness for transition
Coordinates with vendors and clinical staff to ensure necessary durable medical equipment is delivered and set up safely in the patient's home
Engages in interdisciplinary communication, attending care conferences and collaborating with nurses, therapists, and other team members to support holistic care
Provides guidance and support to other home care staff when appropriate to promote safe, effective patient care
Follows infection prevention and safety protocols in the home setting, maintaining awareness of emergency procedures and patient environment risks
Maintains licensure and professional development by completing required continuing education and participating in clinical in-services and training
Position Qualifications
Required:
Graduate of an accredited Physical Therapist Assistant program with an Associate of Applied Science (AAS) degree
Licensed or license-eligible as a Physical Therapist Assistant in New York State
CPR certification (must be obtained within the first three months of employment)
Preferred:
Up to two years of combined education, training, or clinical experience in an outpatient setting
