Medication Coordinator
WSFSSH MISSION STATEMENT

JOB SUMMARY
Under the supervision of the Administrator, provide a range of services and supervision to residents the goal of enabling residents to live permanently as safely, as happily, and independently as possible.
Responsibilities include but are not limited to:
Serve as Person-In-Charge in the absence of Administrator/Director, Assistant Administrator or Clinical Coordinator, including directing the implementation of emergency
Assist with the supervision of the residents and monitoring the facility.
Check in with all department, always assure appropriate staffing.
Assure compliance with policies and Procedures
Coordinate all resident appointments, assure appropriate documentation for these appointments, on and off on-site visits with medical providers.
Supervise on site testing and vaccine clinics.
Discard all discontinued medication monthly and complete required report. Dispense over-the-counter medications as instructed by doctor and according to DOH standards, and record in required log.
Provide and file all medication receipts for discharged residents.
Drop off and pick up medication prescriptions as required.
Alert Manager of Residential Services of resident medication depletion levels, shortages, non-compliance by residents, and medication misuse by residents.
Check to assure that laundry is done in accordance with facility procedures.
Documentation of Medication/Administration:
Assure that all medications are administered according to medication orders and in compliance with agency protocols orders and in compliance with all applicable Department of Health regulations, including assuring prescriptions, inventory, audits, record-keeping, and assistance with administration/monitoring of medications.
When assisting with medication administration, carefully watch each resident, assuring that: The right resident, is taking the right medication, in the right dose, at the right time, by the right route.
Record all prescribed medication that enters the residential facility.
Assure all documentation is completed, recorded timely and meets DOH standards
Manage Medication Changes:
Medication changes, either in writing to the medical provider and coordinate with the pharmacy, and document changes and file accordingly.
Assure that all medication administration is documented by the individual assisting with administration and is in compliance with all applicable regulations. In the event of a medication incident, resident absence, or resident refusal of medications, assure that this incident, absence, or refusal is documented in compliance with all applicable regulations and notify the respective Social Worker and the Administrator. Update medication spreadsheet which records medication being taken by residents. Secure all medication and resident files.
Ensure that the Medication Administration Reports (MARs) is updated and accurate
Other duties as assigned by the Administrator.
Services to Residents:
Supervise the personal hygiene and physical wellbeing of residents through building rounds and task supervision of Personal Care-Housekeeping staff.
Directly provide front desk, personal care, and housekeeping services to residents in absence of assigned Personal Care Aides and housekeeping staff, in case of emergency, or in special circumstances: Assist residents in participating in activity programs and in the life of the facility. Arrange appropriate escorts for residents to medical and other appointments.
Other:
Work with Social Service staff and any other agencies to assist with resident adherence with medication regimen.
Work as a member of a team to establish and maintain high level of care, respect for, and communication with residents.
Work cooperatively with other staff members and communicate effectively with appropriate staff about resident progress, needs, and problems
Attend training sessions and conferences for enhancement of job skills, as required and attend and participate in monthly staff meetings.
Implement emergency procedures, as necessary.
Submit all required reports in a complete and timely manner.
REQUIRED KNOWLEDGE SKILLS & ABILITIES
At least 21 years of age.
Emotionally, mentally, and physically able to perform job responsibilities.
Good computer skills and detail oriented.
Able to speak, read, and write English (as required by DSS regulation 487.9.13).
REQUIRED EDUCATION, EXPERIENCE AND CREDENTIALS
High school diploma or General equivalency diploma required OR 4 years of similar, consistent and verifiable experience.
Minimum of one year experience working with dependent persons.
Preferred: Experience working with elderly persons, homeless persons, and/or persons living with serious mental illness.
PHYSICAL REQUIREMENTS
This role may have physical demands including, but not limited to, lifting, bending, pushing, pulling and/or extended walking and standing. This role may also require uniforms and/or usage of Personal Protective Equipment (PPE).
