Overview
We are seeking a dedicated and compassionate Physical Therapist Assistant to join our nursing home team as Director of Rehab. In this role, you will work closely with licensed physical, Occupational and Speech therapists to provide high-quality care to nursing home residents recovering from injuries, surgeries, or managing chronic conditions. Your contributions will be vital in helping residents regain mobility and improve their overall quality of life.
Duties
Strong organizational skills for maintaining medical records and documentation accurately.
Attend team meetings as scheduled
Assist physical therapists in developing and implementing treatment plans tailored to individual patient needs.
Conduct therapy sessions under the supervision of a licensed physical therapist, ensuring adherence to prescribed protocols.
Monitor patient progress and report any changes or concerns to the supervising therapist.
Maintain accurate and up-to-date medical records for each patient, documenting treatment sessions and outcomes.
Educate patients and their families on exercises, techniques, and home care strategies to support recovery.
Collaborate with facility's multidisciplinary team.
Participate in comprehensive care planning for residents with complex needs.
Skills
Proficiency in medical terminology to effectively communicate within the nursing home setting.
Experience or knowledge in telehealth practices to facilitate remote resident interactions
Understanding of mental health counseling techniques and behavioral therapy approaches is a plus.
Ability to work with special education needs and crisis management strategies when necessary.
Excellent interpersonal skills for building rapport with residents and collaborating with healthcare professionals.
