The Clark Health and Wellness Center is a new state of the art medical facility that will provide high quality, convenient and easy access to all patient's health and wellness services under one roof. Offering access to urgent care, primary care physicians, specialty physicians, rehab, lab, and imaging services for patients in our communities. The Health and Wellness Center will be a multispecialty experience for our patients and team members.

As a member of an interdisciplinary team, the Physical Therapist is responsible for physical therapy evaluation, treatment planning, treatment implementation, documentation and patient/family education. The incumbent also performs other professional duties and participates in department/facility initiatives as assigned.

Education, Knowledge, Skills and Abilities Required :

Graduate of an APTA accredited Physical Therapy education program or equivalent.

Education, Knowledge, Skills and Abilities Preferred :

Experience preferred, entry level acceptable, brain injury experience a plus Computer skills and knowledge of applications such as word, excel, powerpoint preferred

Licenses and Certifications Required :

Physical Therapist License.
