Schedule: Monday - Friday 8 a.m. - 5 p.m.

Hospital: Ascension Borgess - Pipp

Location: Plainwell and Kalamazoo, MI

Responsibilities

Coordinate and facilitate patient admission according to patient acuity, bed availability, and required services.

Perform onsite clinical assessments of potential candidates.

Coordinate transfer process, working with referral sources, insurance companies, and various healthcare staff and departments.

Monitor patient's insurance status during hospital stay.

Assist with development and improvement of processes designed to keep referral sources informed of patient status and discharge recommendations.

Provide pre-admission assessment, insurance verification, and pre-certification for admission.

Requirements

Licensure / Certification / Registration:

Social Worker credentialed from the Michigan Board of Social Work preferred. Any level accepted.

Registered Nurse credentialed from the Michigan Board of Nursing preferred.

Rehab Therapist preferred. Physical Therapist (PT), Occupational Therapist (OT), Certified Occupational Therapist Assistant (COTA), or Physical Therapist Assistant (PTA) accepted from respective Michigan Board of License.

Education:

High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Additional Preferences

3 years critical care experience.

Patient and family care centered experience.

Prior clinical education experience.
