Physical Therapist

The Licensed Physical Therapist (PT) provides specialized physical therapy services to adults with developmental disabilities within Intermediate Care Facility (ICF) group home settings. The PT recommends interventions that contribute to the individual's plan of care, focusing on enhancing mobility, strength, balance, and safety, with the goal of improving functional independence and quality of life. This role emphasizes developing individualized care plans in collaboration with interdisciplinary teams to help each person reach their highest potential . Our organization and funding structure allows our clinical team to build long-term relationships with the people we serve and have creativity with care plans without the complex components around billing. This position would require some travel throughout the day between local programs. The work hours can be flexible based on schedule needs - this is full-time 32 hours per week.

Responsibilities:

Evaluates sensory-motor skills, strength, mobility, balance, posture, and safety related to daily activities. Identifies goals for mobility, self-care, safety, and community participation.

Collaborates with the interdisciplinary team to create individualized, person-centered treatment plans.

Implements therapy interventions (exercise, modalities, mobility training) under a licensed prescriber's direction as/when applicable, monitoring and adjusting based on progress.

Recommends mobility aids (e.g., wheelchairs, walkers) and adaptive devices. Modifies home environments to ensure safety, reduce injury risk, and promote independence in daily living.

Trains direct care staff on proper use of assistive devices, mobility aids, and therapeutic techniques and home exercise regimes. Educates on body mechanics, fall prevention, and safety measures, including teaching new program staff members during orientation.

Assesses physical environments, ensuring safety equipment minimizes fall risks and enhances safety for individuals with disabilities. Recommends necessary modifications.

Maintains accurate, up-to-date documentation in line with policies and procedures and regulatory standards.

Engages in continuing education and stays updated on best practices in physical therapy, particularly for individuals with developmental disabilities.

Prepares for, attends and actively participates in staff and organizational meetings to maintain high standards of care, exchange ideas, and address challenges.

Assists in managing therapy equipment and supplies, ensuring therapy spaces are maintained for optimal care delivery.

Qualifications:

Must hold and maintain valid Connecticut State licensure as a PT.

Must have one year of physical therapy work experience.

Must participate in continuing education activities to promote personal and professional development as per PT CT licensing requirements

Must be able to travel to group homes and other assigned locations.

Experience working with individuals with developmental disabilities (DD) or intellectual and developmental disabilities (IDD) in a clinical, school or community based setting preferred.
