Opportunity Our company is currently seeking a experienced Registered Nurse Case Manager to join our team in providing exceptional patient care. This part-time position is located in Grand Rapids, MI area and will start about 10 hours per week, could lead to full-time. Must have Monday- Friday daytime availability to assist patients. As a Registered Nurse Case Manager, you will play a crucial role in coordinating patient care and ensuring optimal outcomes.

Must be local to the Grand Rapids area and willing to travel locally.

Responsibilities

Strong utilization management skills to assess patient needs and develop individualized care plans

Coordinate with interdisciplinary teams to ensure effective inpatient care transitions

Apply medical terminology and physiology knowledge to enhance patient understanding of treatment plans

Collaborate with healthcare providers in various specialties including OR, hospital medicine, emergency medicine, primary care, and critical care

Advocate for patients' needs and facilitate access to necessary resources for comprehensive care

Qualifications

BSN or Associates and 5+ years of experience

Certified case manager a plus

Active Registered Nurse license in Michigan

Proficiency in utilization management practices and patient care coordination

Strong understanding of medical terminology and physiology

Excellent communication skills and ability to work effectively within a multidisciplinary team

Experience in hospital settings preferred
