Position: Physical Therapist - Home Health

Type: PRN

Responsibilities:

Conduct thorough assessments of patients' mobility, strength, and coordination

Design and implement personalized exercise programs to improve physical function and mobility.

Provide education and guidance on injury prevention and rehabilitation techniques.

Work closely with other healthcare professionals to coordinate patient care

Requirements:

Valid state licensure and certifications for the respective therapy discipline

Drivers License

CPR Certification

Previous experience in home health or a similar setting

Strong communication, interpersonal, and organizational skills

Ability to work independently and manage a flexible schedule

Reliable transportation and willingness to travel to patients' homes
