Position Summary: Care Managers help individuals achieve improved healthcare outcomes. Our staff provide field-based support that assists individuals with medical and behavioral health conditions through the following services:

Assessment Care Planning Connections to Primary and Specialty Care Providers Service Coordination with Treatment Providers Client Education on Diagnoses, Medications and Treatments Linkages to Community-based Resources/Entitlements Benefits:

Salary: $52,000/year
