The Nurse Practitioner position is in the Kohls clinic in Mamakating, NY. You will be working with a team that empowers you to empower your patients!

JOB REQUIREMENTS Master's Degree level Nurse Practitioner program from an accredited college/university New York NP license, BLS Minimum three (3) years primary care practice experience Ability to work independently Working with an Electronic Medical Record (EMR) Active and unrestricted applicable licensure Current CPR (BLS) certification or must be obtained within 60 days of hire NPI & DEA Current National Board Certification May be required to obtain and maintain a valid DOT Medical Examiner Certification, depending on job responsibilities or location.

Shift, Salary and Benefits; This is a Full-Time position, Monday - Thursday, 30hrs/wk., Mon 12 pm- 5 pm, Tues 7:30am-4:30pm, Wed 8am-5pm, and Thurs: 7:30am -5p; $3,000 signing bonus! $123,000 to $125,000 yearly Guaranteed base salary, Yearly bonus eligibility based on quality of care, not RVUs and overall performance of health center, Comprehensive benefit package, 10 Paid holidays, CME paid time off and stipend, 100% reimbursement of licensure/certifications, Malpractice and liability covered, 401k with guaranteed match
