?What Your Day Might Look Like
Conduct comprehensive evaluations and develop patient-centered treatment plans
Collaborate with referring providers, case managers, and fellow rehab team members
Communicate with patients and families to ensure understanding and continuity of care
Support rehab diagnoses and adapt care plans as needed
Principal Accountabilities:
Evaluate outpatients with orthopedic conditions.
Design and implement PT plan of care.
Provide evidence-based interventions.
Document all encounters in the electronic medical record.
Complies with departmental policies/practices.
Maintains patient safety, provides patient/family/caregiver education.
Works cohesively in a team setting.
Minimum Education:
Graduate of an accredited PT program
Minimum Experience:
2-3 years working in an outpatient department preferred
License/Certification:
NJ PT License
