Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Responsibilities This announcement is with the North Florida/ South Georgia Veterans Health System in the Community Reintegration and Recovery Service section, this position is located on site at Gainesville only. The incumbent provides community-based case management services to Veterans participating in the HUD VASH program. The position is responsible for program oversight, monitoring and administration, serving as the primary contact with local Public Housing Authorities (PHAs) and providing case management-based services (directly or through referral) necessary to ensure that Veteran care is coordinated with the VA Medical Center, community health or mental health providers, and agencies that serve homeless populations. Veterans eligible for HUD/VASH come from a wide geographic area. Their health and mental health needs may be complex and require a high degree of clinical oversight and creative problem solving. Services are provided in healthcare settings and in the community. The HUD/VASH Program requires continuous coordination with the community's homeless providers and coalitions as well as with other local, state, and federal agencies. Major duties of the HUD VASH Senior Social Worker can include: Works closely with the VA medical center's homeless programs, residential and community care programs, and domiciliary programs to ensure that coordination between these entities is facilitated and veterans direct care, referral, and follow up is contiguous and based on a continuum of services. Independently provides clinical psychosocial and case management services at an advanced practice level to eligible veterans and their family members in the VA Supporting Housing (VASH) Program to meet biological, psychological, social and environmental needs. Provides support for the Health Care for Homeless Veterans (HCHV)/Community Care Programs including outreach activities, task forces and special assignments. Provides support to the local Medical Center through the assignment of special projects and/or committees. Provides support and mentoring to social work interns and newly hired social workers. Travel to patient homes and outreach sites is required via government vehicle so a valid driver's license is required for this position. GS-12 Senior Social Worker candidates must have the following: Experience/Education: Must have at least two (2) years of experience POST advanced practice clinical licensure and should be in a specialized area of social work practice of which, one (1) year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
