Job Description
This Registered Nurse role in a primary care office focuses on triaging patient calls, providing direct patient care, managing patient charts, and coordinating appointments and follow-ups. The nurse uses an electronic medical record (EMR) system to document care, support care management activities, and ensure patients receive timely, high-quality primary care services.
Responsibilities
Triage incoming patient calls by assessing symptoms, urgency, and appropriate level of care, following clinical protocols and professional judgment.
Document all patient interactions, assessments, and interventions accurately and promptly in the EMR system.
Schedule patient appointments and follow-up visits, ensuring appropriate timing and coordination of care.
Room patients efficiently, including verifying patient information, obtaining vital signs, and preparing patients for provider visits.
Provide direct patient care within the RN scope of practice, supporting primary care providers in delivering comprehensive care.
Participate in care management activities, such as monitoring patient progress, supporting care plans, and assisting with chronic disease management.
Utilize the TS Energy EMR system to access, update, and maintain patient records in a secure and organized manner.
Communicate clearly and professionally with patients, families, and the care team to support understanding of care plans and follow-up needs.
Support a safe, organized, and patient-centered primary care environment by following clinical and office procedures.
Essential Skills
Active Connecticut Registered Nurse (RN) license.
Active Basic Life Support (BLS) certification from any recognized organization prior to starting.
Demonstrated triage experience, including assessing patient needs over the phone or in person.
Proficiency in rooming patients, including collecting vital signs and preparing patients for provider evaluation.
Experience utilizing electronic medical record (EMR) systems in a clinical setting.
Ability to provide patient-centered care and support care management activities in a primary care environment.
Strong communication and documentation skills to ensure accurate charting and clear care coordination.
Additional Skills & Qualifications
Experience with the TS Energy EMR system or similar EMR platforms.
Prior experience in a primary care office or outpatient clinical setting.
Ability to manage multiple tasks such as triaging calls, charting, and scheduling in a fast-paced environment.
Strong organizational skills and attention to detail when managing patient records and appointments.
Commitment to collaborative, team-based care and continuous improvement in patient services.
Work Environment
This role is based in a primary care office that operates Monday through Friday from 8:00 a.m. to 5:00 p.m. The nurse works in a clinical setting that involves direct patient interaction, phone-based triage, and extensive use of the TS Energy EMR system. The environment supports a structured weekday schedule with no listed weekend hours. Pay and Benefits
The pay range for this position is $35.00 - $40.00/hr.
