Summary This Licensed Practical Nurse position is located at the Cumberland County VA Clinic in Mechanicsburg PA to assist in Primary Care. Specialties and Phlebotomy skills required. Prepares patients for treatments and examinations and positions them as required. This will involve placing patient on examining table and chairs and exposing area to be observed and/or treated by the physician. Observes patients for any immediate needs and gives prompt attention or seeks help as necessary. This can be of an emergent nature or relate to the immediate comfort of the patient. Participates in nursing in-service educational programs. Takes vital signs, weights, collects specimens, and makes other pertinent observations specific to the primary care area. Sets up the examination rooms by providing necessary instruments and supplies, seeing that the area and equipment are aseptically clean and in order, assuring that patient charts, x-rays, and consults are available. Assists physicians by supplying needed equipment providing information such as vital signs, weights, observations, and information obtained from the patient. Assures smooth clinic operations by directing the patient to appropriate areas. Inventories, orders, and restocks needed linen and supplies. Reports equipment in need of repair. Detects and reports unsafe work conditions. Assures orderliness and cleanliness of all storage and work areas. Applies body substance isolation precautions to all areas of patient care. Receives patients scheduled for various clinics or arriving for other ambulatory care services. Explains procedure to patients and assist them during examination. Administers life support measures cardiopulmonary resuscitation (CPR) when required. Performs messenger duties for Primary Care as needed to obtain stat medications, equipment, specimens, etc. Performs electrocardiogram (EKG) and glucometer checks. Administers prescribed medications, per policy. Documents procedures performed and results. Documents teaching/instructions and response in proper format, utilizing documentation standards. Documents vital signs, weights, and special observations using proper format. Identifies patient and family learning needs, educates the patient/family member(s) according to the patient education record, and documents and evaluates learning. Instructs the patient and family on his/her role in various procedures performed. Explains how equipment will be used. Reinforces information previously supplied by the physician or nurse. Informs nurse case manager of obstacles to learning that will require enhancement. Maintains a safe and secure environment for patients. Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations. Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant] patient information. Completed work should need only a general review by a registered nurse (RN) or physician (MD/DO) for appropriateness and conformity with established policies/procedures. Ability to observe, identify and respond to the patient's needs for care, including medication, equipment-assisted care and patient/family education. In organizing and delivering care, the LPN/LVN recognizes and considers emotional, socio-economic, and age-related factors. Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures. Serves as a preceptor in orienting, educating, and training less experienced LPNs/LVNs or NAs/HTs related to support duties for these more complex, specialized tests/procedures.
