Summary The Clinical Reviewer works closely with VISN and Medical Center Leadership. The Clinical Reviewer develops and uses collaborative relationships with the other Medical Center disciplines to enhance the delivery of patient care to Veterans and provide educational experiences for the staff. Responsibilities The Clinical Reviewer independently performs assigned functions using sound administrative management principles, performance improvement methodologies, and current concepts from research and studies as a framework for practice. The Clinical Reviewer interfaces with hospital leadership on a regular basis regarding issues that arise in the program and works closely with the facility Risk Manager and the risk management program. Functions or Scope of Assigned Duties: Has oversight of the Peer Review for Quality Management program and all its related activities. As a result of referrals from the various organizational surveillance programs, screens and evaluates clinical events against established criteria for Peer Review referral. Prepares all clinical summaries of all cases to be presented to the Peer Review for Quality Management committee. Provides Peer Review "Just in Time" Training. Identifies appropriate staff for completion of initial review(s) and committee membership in consultation with Chief of Staff and/or Service Chief. Monitors and tracks the various due dates, level of care assignments, and level of care assignment changes to ensure timely submission of the various required reports. Completes data analysis to include but not limited to completion dates, levels of care, level of care changes, aspects of care, source of event review, and systems issues Oversight and coordination of reviews sent to the Peer Review Committee for final level of care assignment. Responsible for ensuring complete and timely minutes related to Peer Review Committee. Develops competencies in team function. Evaluates the Peer Review process as it functions in the facility and responds to improve as appropriate and as delineated by VHA Directive. Develops and submits the quarterly and yearly Peer Review for Quality Management Report to the executive leadership team. Coordinates and submits Peer Review rosters to the Office of Risk Management for quarterly external audit. Coordinates the submission of cases to the Office of Risk Management for facility requested reviews. Monitors committee related actions and outcomes to closure. Ensures system wide issues are assessed and if identified, the respective actions and outcomes are tracked to closure. The incumbent must have knowledge of programs, missions and organization of complex medical center facilities and the inter-relationship between health care delivery systems in the Department of Veterans Affairs and the US at large. Ability to analyze problems and present both written and oral recommendations taking into full consideration the wide range of factors and requirements, which affect the management of the health care delivery system. Collaborates, coordinates and participates along with the Risk Manager and the Chief of Staff the disclosure of adverse events to patients and/or their next of kin. Reviews incidents and makes recommendation as to the need to proceed with an Institutional Disclosure. Coordinates training as necessary regarding the Disclosure of Adverse Events process in accordance with the VHA guidelines. Assists with visits from regulatory and licensing agencies to include the Office of the Inspector General (OIG) and The Joint Commission. Conducts reviews of policies, procedure and VHA guidelines requiring, judgment and interpretation to recommend prioritization of Performance Improvement. Qualifications Basic Requirements: English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. An ADN or Diploma in Nursing AND 2 years of professional nursing experience in which one year is equivalent to Nurse I, Level 2; OR a BSN and 1 year of professional nursing experience equivalent to the Nurse I, Level 2; OR a Master's degree in nursing (MSN) and no additional professional nursing experience; OR a Master's degree in a related field with a BSN and no additional professional nursing experience.
