Physical Therapist (Home Health)

Location: Redding, CA

Job Type: Full-Time

Schedule: Monday - Friday | 8:00 AM - 5:00 PM

Compensation: $135,000 - $149,000 annually (Pay Per Point Model)

Sign-On Bonus: $20,000

Are you a compassionate Physical Therapist looking to make a meaningful impact in patients' lives? Adecco Healthcare is partnering with a leading home health provider to identify a dedicated Physical Therapist to join their growing team serving the greater Redding area.

This is an excellent opportunity for a clinician who enjoys working independently while helping patients achieve greater mobility, independence, and quality of life in the comfort of their own homes.

Position Summary

The Physical Therapist is responsible for evaluating and treating patients in a home health setting while collaborating with an interdisciplinary care team. This role develops individualized treatment plans, provides patient and family education, documents care in accordance with regulatory standards, and manages a caseload that meets productivity expectations.

Responsibilities

Perform comprehensive patient evaluations and functional assessments.

Develop individualized treatment plans based on physician orders and patient needs.

Provide therapeutic interventions to improve mobility, strength, balance, and functional independence.

Educate patients and caregivers on treatment plans, home exercise programs, and safety techniques.

Communicate patient progress and recommendations with physicians and interdisciplinary team members.

Maintain accurate and timely clinical documentation.

Modify treatment plans based on patient progress and changing clinical needs.

Participate in care conferences, team meetings, and ongoing professional development.

Manage patient caseload while meeting agency productivity standards.

Qualifications

Master's Degree in Physical Therapy from a CAPTE-accredited program.

Current California Physical Therapist license in good standing.

Minimum of 9 months of Physical Therapy experience.

Home Health or community-based experience preferred.

Current CPR certification.

Valid driver's license, reliable transportation, and ability to travel throughout the assigned coverage area.
