Provider and Nursing Services for Public Health Clinics
Program Descriptions
The Monroe County Department of Public Health provides public health services to Monroe County in a variety of venues. The goal of the Department is to provide essential health care services to the residents of Monroe County in the areas of disease surveillance, clinical services and preventive health education in order to prevent and control the spread of communicable disease. Provider and nursing services are provided in the Starlight Pediatric Program (Foster Care Pediatrics), the TB Control Program, the Immunization Program, the Disease Control Unit, the HIV/STD Control Program, and the Food Safety Program.

Previous pediatric experience and experience immunizing pediatric patients and/or experience in correctional facilities is preferred
Locations and Job Descriptions :
Starlight Pediatric (Foster Care) : 451 East Henrietta Road, Rochester, NY 14620. The Starlight Pediatric Clinic provides primary pediatric care and nurse case management services to the high risk, medically and psychosocially complex population of children in the foster care system. These services are provided by nurse practitioners, physician assistants, nurses (RN and LPN), a medical director and support staff. More than 3,000 visits are conducted annually. Staff interacts with the Department of Human Services (DHS) and other pediatric specialty providers. This program operates Monday-Friday from 8:00 AM to 5:00 PM . Provide staff for two evening clinic sessions per week (until 7:00 PM) and up to two Saturday morning sessions per month.
Tuberculosis Program: 111 Westfall Road, Rochester, NY 14620, and a variety of community settings including patient homes. The mission of the TB Program is to prevent and control the spread of tuberculosis in Monroe County. To carry out this mission, the program provides specialty clinic services, directly observed therapy (through home visits) and case management to those with suspected or active disease. In addition, the program screens high risk individuals and groups, and investigates contacts of those identified with tuberculosis. The program also provides education to the community. The program is staffed with nurses (RN and LPN), outreach staff, contract physicians, an x-ray technician and support staff including clerical staff. The program contracts with private laboratories to process specimens and interacts with many community-based organizations, local physicians, hospitals and other care providers. The clinic provides up to 1000 specialty physician clinic visits, 1000 x-ray visits, and 3000 nursing clinic visitsfor medication refills each year. In addition, there are 750 Interferon Gamma Release Assay (IGRA) blood screenings and 1,000 PPD screenings (in-clinic and off-site.) Outreach workers conduct approximately 8,000 Directly Observed Therapy (DOT) visits each year. This program operates Monday, Tuesday, Thursday and Friday, 8:00 AM to 5:00 PM; Wednesdays, 8:00 AM to 7:00 PM.
Immunization Program : 111 Westfall Road Rochester, NY 14620 and a variety of community settings including, but not limited to, community provider offices, daycare centers and providers and schools in Monroe County and the Monroe County Jail. The mission of the Immunization Program is to prevent vaccine preventable disease and promote equal access to immunization to all County residents. This mission is carried out through clinic services, both on-site and off-site, outreach services and education. The clinic is staffed by nurses (RN and LPN), with support from the clerical staff. The clinic provides over 10,000 vaccines to approximately 4,700 patient visits each year. These include routine Immunization clinics, Back to School clinics, school based Flu vaccine clinics, outreach clinics to high-risk individuals, and outbreak mitigation by immunization clinics. Staff interacts with many providers, school health programs, community day care providers, and the Monroe County Medical Society. T his program operates Monday through Friday from 8:00 AM to 5:00 PM. Additional hours are required during seasonal high volumes, outbreaks and other special projects.
Disease Control Unit : 111 Westfall Road Rochester, NY 14620 and a variety of community settings including, but not limited to, community provider offices, restaurants, daycare centers or providers and schools in Monroe County and the Monroe County Jail. The goal of this program is to prevent the transmission of reportable communicable disease by means of surveillance, investigations, intervention, education and research. Services are provided by nurses (RN and LPN), public health assistants, epidemiologists and clerical staff. Investigations may include extensive phone interviews of both patients and providers and visits to homes, restaurants, schools, day cares and other community sites. On average, up to 1,500 investigations and up to 75 community visits are conducted each year. The extent of each disease investigation and the magnitude of the associated contact investigation vary depending on the disease and the individual situation. Related teaching and prevention may include distribution of educational materials, releases of health alerts, and visits to schools, day cares or other community settings. This program operates Monday through Friday, 8:00 AM to 5:00 PM with additional hours (as needed) during outbreak situations.
HIV/ STD Control Program : 111 Westfall Road, Rochester, NY, 14620, 855 West Main Street, Rochester, NY and a variety of community settings including, but not limited to, patient homes. The goal of this program is to prevent the transmission of reportable sexually transmitted diseases (HIV, Syphilis, Gonorrhea and Chlamydia) through diagnosis, treatment, contact identification and treatment, education of clients and providers, surveillance and linkages to care. Nurses (RN and LPN), Public Health Assistants and clerical staff provide investigation, screening, community engagement and navigation and linkage services with contracted clinicians (MD, NP/PA) and community clinicians. Services are provided by phone, in home visits, clinic visits and visits to other community based settings. This program operates Monday through Friday, 8:00 AM to 5:00 PM.
Children and Youth with Special Health Care Needs Program (CYSHCN): 111 Westfall Road, Rochester, NY 14620 and a variety of community settings including provider offices and community events. This program provides information and referral services to families of children with a serious or chronic health related condition that impacts their health or development. This program operates Monday through Friday, 8:00 AM to 5:00 PM.
