Summary The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their individual scope of practice to directly care for patients at the Clement J. Zablocki VAMC in Milwaukee, WI. A CPS functions as a member of their assigned team(s) and serves as a mid-level provider to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. Responsibilities The individual scope of practice will be dependent upon the incumbent's education, training, and experience required to perform the functions of the position. All VHA pharmacists with a SOP that includes prescriptive authority must participate in a professional practice evaluation program. Major Duties of the Internal Medicine Clinical Pharmacy Specialist include: Participates in medical team rounds on daily basis. Participates in multidisciplinary team rounds as scheduled. Provides primary cross coverage to acute care CPS team including Critical Care and Cardiology CPS which includes processing inpatient pharmacy orders for all ICU patients. Provides cross coverage to nutrition support CPS. Provides consultation on selection of appropriate dosage regimens as follow-up when non-therapeutic drug levels are obtained for medical center patients. Provides review of patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. Monitor and report drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures. Applies knowledge of normal laboratory values in the evaluation of patient care, recognizes significant abnormalities, and makes dose adjustment recommendations based on objective laboratory findings. Performs medication histories with admitted patients face-to-face whenever possible and performs medication reconciliation for patients all patients admitted to team. Provides discharge medication reconciliation and consultation to patients upon discharge from assigned medical team. Reviews and evaluates prior authorization drug requests (PADRs) for appropriateness and compliance with established criteria where applicable. Assists Pharmacy, Nutrition and Therapeutics (PNT) committee with medication use evaluations and other safety initiatives, as needed. Promotes and monitors compliance for established drug therapy policies. Works with providers to ensure compliance with national, VISN, and local initiatives.
