The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.
Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. Norfolk CSB seeks team members interested in providing skilled services in a public service environment. The NCSB is seeking a Case Manager IV to serve as the agency liaison to provide discharge planning and intensive case management for consumers currently hospitalized in DBHDS State Hospitals. Acts as the agency representative in legal proceedings for recommitment hearings. Provides discharge planning services for clients currently ready for discharge in the State Hospitals. Completes and follows IDAPP consumers in the community on a limited basis in collaboration with other providers. The case manager may also carry other cases on an interim basis; given the numbers of clients in the community. Completes clinical documentation in accordance with agency guidelines.
Department Hiring Salary Range: $59,745-$68,559
Essential Functions
Essential functions include but are not limited to:
Provide discharge planning to consumers in local or diverted DBHDS State hospitals.
Monitor provision of treatment throughout the hospitalization or stay to ensure utilization of effective treatment modalities.
Establishes and maintains collaborative relationships and ongoing communication with providers.
Serves as NCSB agency liaison for DBHDS State hospitals when working with staff and care coordinators for developing appropriate discharge plans.
Collaborates with DBHDS State hospital staff and community providers to identify and overcome barriers to timely and effective discharge planning.
Coordinate case reviews with the Program Administrator, Prescription Team, DBHDS State Hospital Treatment Team, Census Management Meetings, and/ or Community Care Providers as appropriate.
Links consumers to community resources, services, and supports specified in discharge plan.
Completes IDAPP plans for consumers hospitalized in DBHDS State hospitals.
Identify systems issues which may contribute to an increased length of stay.
May manage a caseload.
Maintain accurate clinical, statistical, and financial records in accordance with Norfolk CSB standards.
Maintains chart documentation in compliance with agency guidelines and state regulations and completes an accurate record of direct and consumer related services provided.
Complies with policies and regulations for HIPAA, mandated reporting of abuse and neglect, patient confidentiality, patient rights and other confidentiality standards related to the handling of clinical records.
Performs other related duties as assigned.
Education/Experience
Bachelor's degree in a human services field.
Considerable experience in a human services field related to the area of assignment. Some assignments may also require specific experience working with various @types of client populations; master's degree preferred in some areas.
Preferred Education/Experience:
Master's degree in human services or a related field with at least two years of mental health experience, or possess a Bachelor's degree with equivalent experience.
Experience in quality assurance, discharge planning, case management and working with adults with serious mental illness and co-occurring disorders.
Additional Information & Requirements
Valid Driver's License may be required depending on assignment.
Certification in basic first aid and CPR, depending on area of assignment.
Maintain QMHP certification preferred.
Work Hours: Monday-Friday 8:30 AM - 5:00 PM

Retirement
If you are hired October 5, 2010, or after to a position with City of Norfolk Retirement System benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.
If you are hired January 1, 2022, or after to a position with Virginia Retirement System (VRS) benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.
The Tuition Assistance Program is established to encourage employees toward continued self-development and education. Permanent full-time and permanent part-time classified, unclassified permanent, full-time special projects, and constitutional employees who have completed six months continuous service will be eligible to apply. The applicant's school of enrollment must be an accredited institution.
