Integrated Care Coordinator (Medical Assistant)
M-F 8am-5pm
Pay- $27/hr
The Medical Assistant (MA) Integrated Behavioral Health (IBH) Care Coordinator is responsible for attending to Primary Care patients in the Health Center setting for behavioral interventions for a variety of patient issues, including depression, anxiety, pain management, and other chronic medical conditions for which the patient would benefit from behavior changes, coaching, or additional support. The MA IBH Care Coordinator may also intervene for crisis response or to support the patient and care teams in establishing behavioral norms for the continued care relationship.
The MA IBH Care Coordinator provides psychoeducation and case management follow-up and referral to improve patient health outcomes. The MA IBH Care Coordinator directly supports the Behavioral Health providers, including Behavioral Health clinicians (Psychologists, LCSW, LMFT) as well as the Psychiatric Nurse Practitioners, performing vitals and conducting case management around medication monitoring, behavioral activation, maintenance, and linking patients with resources in the community.
The MA IBH Care Coordinator contributes equally to the patient care teams through huddles, case consultation, patient advocacy, mental health education, and collaborative team meetings with medical staff. The MA IBH Care Coordinator is expected to be professional, punctual, maintain regular attendance, and be cooperative, organized, and enthusiastic at all times.
Required Qualifications
High School Diploma or equivalent.
Must have a Medical Assistant training certificate of completion.
Current CPR or CPR/AED certification required.
Experience and/or education in human-related fields such as social work, psychology, counseling, alcohol or chemical dependency.
Bilingual in Spanish preferred; or other languages desirable.
Demonstrated commitment to providing services for the underserved and sensitive, working with various people from low-income populations with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.
Communicate clearly and concisely, both verbally and in writing, and demonstrate high listening skills.
Ability to establish and maintain positive, professional relationships.
Exceptional interpersonal skills.
Excellent customer service experience, including active listening, problem-solving, and calmness in emotional or stressful situations.
Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
Ability to maintain detailed records.
Compliance with all state and federal laws and regulations pertaining to the position, including Computer (PC) literate in Microsoft Word, Excel, PowerPoint, Outlook email and database software.

Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
Assist and support the Center/Department/Program to meet standards of High Reliability.
Must be willing to successfully complete the credentialing process.
Essential Responsibilities
Conducts warm hand-offs between the patient and referring primary care provider during health center hours to link patients with Behavioral Health Services.
Assists patients in navigating the mental health system and appropriate linkage to various county agencies and other community resources based on patient needs and availability.
Monitors and implements Patient-Centered Health Home (PCHH) goals.
Monitors and implements Certified Community Behavioral Health Center (CCBHC) goals.
Performs medical assistant duties, prepare rooms, and room patients, record medical histories, vital signs, perform immunizations, and medication assistance.
Documents all patient interactions in the Electronic Health Record according to policies and timelines.
Assists with related data collection, related projects, initiatives, utilization, and review activities.
Travels when necessary to support operational needs, meetings, and any function requiring presence.
Maintains immunization files for the county registry.
Maintains and adheres to confidentiality and privileged communications (patient, employee, and organization).
Performs duties as assigned within the medical assistant's scope of practice.
Performs other duties as assigned.

Pay and Benefits
The pay range for this position is $25.00 - $25.00/hr.
