The Community Health Nurse providers observation and guidance to community nursing home (CNH) staff on quality of care, quality of life and specific Veterans Health Administration (VHA) programs that are helpful to Veterans. This nurse makes the required nursing visits (per VHA Handbook 1143.2) in VA contracted extended care facilities and complete the necessary preparation, follow-up, and documentation to the visit. Follow-up phone calls, coordination of care, interfacing with Medical Center professionals as well as community providers is expected. Frequent travel is required throughout the VAIHCS service area. This nurse is self-directed in providing quality oversight management to Veterans in the program and demonstrates that she/he is a motivated individual who works well on her/his own. This nurse demonstrates the ability to work as a member of an interdisciplinary team. This nurse works effectively with patients, families, and contract facility staff and ensures that progress notes from patient visits are entered, and other required data is input into program databases/spreadsheets for tracking and trending purposes. Reviews and analyzes the inspection findings of other agencies, as they apply to contracted facilities. Participates in annual inspections of contracted facilities and assists in the renewal of contracts. Identifies appropriate community facilities with which to initiate contracts, when needed. This nurse coordinates non-skilled in-home services and conducts functional assessments using national tools. Qualifications Basic Requirements: English Language Proficiency.
