As a Social Worker 1 with the Department of Human Services, Wernersville State Hospital, you will help individuals navigate important steps in their treatment and recovery. DESCRIPTION OF WORK

This position provides professional social work services to support consumers with mental health needs and to guide their treatment, planning, and continuity of care. As a Social Worker 1, you will perform the following duties:

Consumer Support: Provide age appropriate and culturally sensitive services that meet individual needs

Resource Coordination: Link consumers with community systems, benefits, identification documents, and supportive programs

Treatment Participation: Contribute information to treatment teams, complete assessments, and support discharge planning

Regulation Compliance: Interpret and follow federal, state, and departmental regulations related to documentation and service delivery

Family Communication: Establish connections with families, county liaisons, and community providers to support continuity of care

Case Documentation: Prepare notes, reports, and updates to support treatment goals and maintain consistent care

Work Schedule and Additional Information:

Full-time employment

Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. May change based on operational needs.

Overtime and travel as needed

Telework: You will not have the option to telework in this position.

Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

A bachelor's or master's degree in social work or social welfare.

Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.

Other Requirements:

You must be able to perform essential job functions.

Legal Requirements:

A conditional offer of employment will require a medical examination and drug screening.

This position falls under the provisions of the Older Adult Protective Services Act.

Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
