Summary The Health Promotion and Disease Prevention (HPDP) Program Manager will serve as a facility content expert for HPDP and oversee the integration of HPDP services into patient care throughout the facility and affiliated community-based outpatient clinics. The HPDP Program Manager is required to perform statistical reporting, evaluation, consultation, and collaboration with non-Department of Veterans Affairs (VA) facilities as well as other VA health care professionals and administrators. Responsibilities The Health Promotion and Disease Prevention (HPDP) Program Manager will serve as a facility content expert for HPDP and oversee the integration of HPDP services into patient care throughout the facility and affiliated community-based outpatient clinics (CBOCs). The HPDP Program Manager is required to perform statistical reporting, evaluation, consultation, and collaboration with non-Department of Veterans Affairs (VA) facilities as well as other VA health care professionals and administrators. The incumbent must be able to comprehend and maintain knowledge of current VA guidelines, policies, Federal regulations, and laws governing medical benefits for Veterans. The HPDP Program Manager must be capable of providing strong leadership. Serves as an educational resource for issues related to evidence based HPDP. Routinely assesses the prevention-related educational needs of both patients and employees; develops educational strategies to meet those needs appropriately and effectively. Collaborates in the development, delivery, and evaluation of educational programs and learning outcomes directed towards both Veterans and staff. Recommend and implement changes at the program or service level to meet HPDP standards. Responsible for strategic planning, program development and implementation, monitoring and evaluation of HPDP programs and services in the facility. Ensures that HPDP strategic plan aligns with national goals and is integrated into Primary Care and facility strategic plan. Partners with Primary Care, the HPDP Committee, Health Behavior Coordinator, Whole Health, Patient Education, and System Redesign/HRO to assist with HPDP- related facility initiatives and utilizes data to support initiatives, including:COVID-19 Preventive Health Inventory Office of Quality and Patient Safety performance measurement tools (e.g., SHEP and MAT) Community Administration of Vaccine Encounters (CAVE 1.2), SAIL performance measures Press Gainey survey, complies with the VA Medical Center's Research Policy. Apply relevant research findings to specialty nursing practice. Critically evaluates research and incorporates findings appropriately. Disseminates relevant research findings through practice, education, and consultation. Organize and share findings appropriately from surveillance activities. Effectively participates in organization's interdisciplinary approach for performance improvement strategies. Occupation-specific duties and responsibilities regarding the delivery of selected HPDP services may be assigned locally as appropriate. Offers mandatory HPDP staff training, coordinates logistics of trainings and enrollment, and ensures compliance with training. Incorporates training in evidence based HPDP into new employee orientation for Primary Care and other relevant nursing staff. Attends interdisciplinary team meetings to provide consultations on promotion of preventative health behaviors. Maintains professional knowledge and skills based on current published or communicated information including professional readings, referenced nursing journals, workshops, and seminars. Ensures that educational opportunities are provided to clinical and administrative staff members to support and enhance their involvement in and delivery of local HPDP programs, services, and activities. Develops or coordinates the development of user-friendly tools to assist in successful implementation of HPDP programs and services. Qualifications Basic Requirements: English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Nurse II Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others.
