Summary This position is located in the Community Care Service Line at the Tomah VA Medical Center. The Tomah VAMC is a complexity level 3 facility. The primary purpose of the position is to serve as a Community Care Coordinator. working collaboratively with the patient care teams as well as a variety of specialty health care providers. Responsibilities Medical Appointment and Support Responsible for generating authorizations for pre-authorized medical care, while making eligibility determinations for the various programs in Community Care. Acts as a liaison between the veteran and the vendors with regard to Community Care and VA clinical services and/or primary care services. Obtains requested reports, x-rays, labs and any pertinent information required to complete the Community consult in CPRS. Ensures all necessary components and documentation are distributed to appropriate services at the private institution where service will be provided. Screen/receive phone calls in a courteous and timely manner, determine the nature of requests and provide the information desired using privacy rules and established processes. Reviews records for administrative criteria ensuring compliance with established standards. Makes administrative recommendations and program specific decisions that promote the efficient use of resources with the goal of improving timeliness and appropriateness of services provided. Acquire knowledge of internal organizational procedures, knowledge of various facility services, knowledge of commonly used medical terminology and abbreviations, and VA acronyms and their meaning. Coordinates the completion of Community Care consults assuring the corresponding report is linked to the appropriate consult, progress note title and patient. Work to resolve issues with veterans, vendors and clinical staff to ensure appropriate care is scheduled and authorized at Non-VA facilities. Eligibility and Administration Reviews and determines veteran's eligibility for specialty programs in the Community Care program (CC) and appropriately processes their authorizations. Contributes to the revenue collection process by identifying patients with third party insurance. Performs other duties as assigned. Examples of specialized experience would typically include, but are not limited to: Generating authorizations for pre-authorized medical care, obtains requested reports, x-rays, and labs, coordinate transportation for Veterans as needed, updating and verifying demographic information, verifying insurance information, data entry, managing consultations, and notifying the patients of his/her schedule appointments.
