Summary The Pharmacy Supervisor, Anticoagulation/Geriatrics Clinical Pharmacy Programs, is responsible for the professional and administrative management of an assigned area in pharmacy service, plays a defined role in budgetary execution, and serves as a consultant and midlevel provider who functions to initiate modify or discontinue medication therapy. Duties include and are not limited to: Supervisory Functions: Manages the activities of assigned Clinical Pharmacy Staff and has the oversight of ongoing inpatient clinical pharmacy services. Works under the supervision of the Associate Chief, Clinical and Education Services and, as calibrates with the Pharmacy Chief and other Associate Chiefs. Works with other pharmacy staff to assure that pharmacy clinical services are well coordinated and incorporated into the overall approach to patient care. Assures that the pharmaceutical care needs of patients are met across the continuum of care within the scope of their clinical specialty area. The incumbent has full administrative authority over subordinates in his/her area of specialty and ensures that services provided meet or exceed the professional and legal standard. Manages people, maintains effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals. Ensures implementation of performance standards, assures that clinical practice is compliant with all regulatory and accrediting body requirements. Clinical Functions: Coordinates all clinical pharmacy functions for his/her assigned practice area, helping to achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams. Attends and participates in team meetings, conferences, and other scheduled events as required. Designs, implements, assesses, monitors, and documents therapeutic plans utilizing the most effective, least toxic, and most economical medication treatment for patients consulted to his/her care and other clinics as designated by supervisor. Works independently and effectively to evaluate prescriptions and/or provider orders for accuracy, eligibility, availability, and cost effectiveness and therapeutic appropriateness; contacts physicians and makes authorized changes in orders or prescriptions as required. Demonstrates recognized expertise in the assigned area of practice for the purpose of management of several disease states specific to his/her area of clinical specialty. Manages all patients receiving anticoagulation therapy in all medical settings if applicable to his/her clinical practice specialty unless otherwise requested by the attending physician. May also participate as a mid-level provider in the outpatient Anticoagulation Management Clinic.
