Travel Home Health Physical Therapist

A Travel Home Health Therapist (PT) role is now available through Adecco Healthcare and Life Science. In this role, you will provide patient care in a home health setting across multiple communities, including developing individualized plans of care to be followed by the patient, caregiver, and Physical Therapist Assistant (PTA) during subsequent visits. You will collaborate with physicians to address patient needs and ensure appropriate care and actively participate in weekly interdisciplinary case conferences to support recertification and discharge planning. Additional responsibilities include all aspects of home health physical therapy, ensuring high-quality, patient-centered care.

Location and Coverage Areas: Shrewsbury, Westborough, Northborough, Bolton, Hudson, Marlborough, Framingham, MA area

Job duration: Minimum 3-month temp/contract role (potential for extension)

Shift: Monday-Friday 8:00am-4:30pm

Salary: $ 50.66/hr.

Schedule & Compensation

Shift: Monday-Friday, 8:00 AM - 4:30 PM

Weekly Guarantee: 32 hours (with opportunity for up to 40 hours based on patient volume)

Productivity Expectations:

26 points for 32 hours

33 points for 40 hours

Mileage Reimbursement: Yes (between patient visits, from first to last visit)

On-Call/Weekend Requirement: None (flexibility available for schedule adjustments as needed)

Key Responsibilities

Perform comprehensive patient evaluations and develop individualized treatment plans

Provide skilled physical therapy services in accordance with physician orders and care plans

Monitor patient progress and adjust treatment plans as needed

Complete accurate and timely documentation, including OASIS assessments

Collaborate with interdisciplinary care teams to ensure coordinated patient care

Educate patients and caregivers on treatment plans, exercises, and safety measures

Maintain compliance with all regulatory and clinical standards

Required Qualifications

Active Physical Therapy (PT) License

Home health experience

OASIS experience

Preferred Qualifications

Strong interpersonal and communication skills

Ability to work independently and manage time effectively

Patient-focused approach with strong clinical judgment
